Job Summary

The Greater Nashville Regional Council (GNRC) is seeking an experienced social worker to join the Home and Community-Based Services (HCBS) team within its Department of Aging and Disability Services. The Department’s responsibilities include 1) planning and coordination of state/federal investments into programs and services aimed at improving the quality of life for senior adults, 2) coordinating services among a network of senior centers and providers, 3) providing counseling/assessments to help Middle Tennesseans enroll in programs and services, and 4) monitoring the performance of grant-funded programs to ensure compliance with the state and federal requirements.

About GNRC: GNRC is the regional council of governments for Nashville and surrounding counties and serves as one of nine regional economic development districts established by the Tennessee General Assembly in 1965. GNRC also serves as the region’s state/federally designated Area Agency on Aging & Disability (AAAD), working in close partnership with the TN Commission on Aging and Disability (TCAD) and TennCare. Job Responsibilities

The HCBS team provides case management and coordinates a range of social services to eligible seniors and adults with a disability. These programs are intended to empower seniors, disabled adults, and other targeted populations to remain independent in their home and/or community for as long as possible. The programs focus on connecting clients with transportation, meal preparation, housework, personal care, and other supportive services.  The primary duties of the Case Manager include:

  • Effectively manage a client caseload to ensure timely assistance to older adults and their caregivers,
  • Conduct home visits to assess client eligibility for various programs and services,
  • Ensure timely, accurate, and complete documentation,
  • Keep informed on aging and disability issues, available resources, and local service providers,
  • Maintain compliance with professional, program, and industry standards,
  • Cross-train with other Aging and Disability programs at GNRC,
  • Other projects as requested by the program manager.

Qualifications

Qualifications: The ideal candidate will have a Bachelor’s degree in Social Work or related field and experience in geriatrics and case management with excellent verbal and written communication skills and the ability to work within a team setting.  

Required Technology Skills:  Proficiency in Microsoft Office and Adobe Acrobat software is required.  

Travel Expectations:  This position will require travel throughout GNRC’s thirteen-county region. A valid driver’s license and access to personal transportation is required.

Remote Work Flexibility:  This is not a remote position.  The selected candidate will expected to work onsite on core business days.  Work-from-home flexibility is provided when possible at the discretion of the Manager.

Compensation and Benefits:  The minimum starting pay will depend on experience and qualifications.  GNRC offers a full benefits package along with a fully paid pension through the Tennessee Consolidated Retirement System and provides an optional 401k with a dollar-for-dollar match of up to 3% of gross salary.  Inquire for more information about benefits.
