Job Summary

The MAPS Coordinator will be responsible for building and growing a new program that is geared to support individuals with disabilities.  Map is a program that will set people with intellectual and developmental disabilities on a path to learn skills to help them work, live independently, learn about their neighborhoods, and find hobbies that interest them. MAPs service gives people in the program the tools, technology, and support they need to meet the goals they set for themselves.  This is a full-time position.  

  • The Maps Coordinator will have the option of working from home.
  • The Maps Coordinator will have the option of flexing his or her work hours on a day-to-day basis, depending on the need at the time. Start and end times will vary based on meeting schedules, visits, training responsibilities, etc.

Job Responsibilities

MAPS Coordinator Functions:

  • Projects an image that reflects favorably on The Arc Davidson County & Greater Nashville

  • Knowledgeable in and compliant with all policies and procedures of The Arc Davidson County & Greater Nashville

  • Coordinate intake and referrals/process
  • Educate individuals about Home: Home Safety (Ind Coaching) and personal Hygiene (Ind Coaching)
  • Educate the person (and guardian, conservator, and/or family, if applicable) regarding income reporting requirements for public benefit programs, including the Social Security Administration.  Financial Management.
  •  Educate the person with Health Mgmt. Help individuals complete their eligibility and enrollment forms
  • Adhere to federal and state regulations
  • Educate the person with Work: Pre-employment, employment innovation, career development, and independent achievement of employment.
  • Educate the person with Community: Independent travel, community relationships, community activities, and VCRM resources.
  • Conduct comprehensive assessments
  • Leads Person Centered Mapping process
  • Develop and Monitor Person Centered Plans
  • Review progress toward outcomes

Qualifications

Skills Needed:

  •  A Bachelor’s degree from an accredited college or university in a human services field.
  •  A Bachelor’s degree from an accredited college or university in a non-related field and/or one (1) year of relevant experience.
  •  An Associate degree plus two (2) years of relevant experience.
  •  High school diploma or general educational development (GED) certificate plus four (4) years of relevant experience.
  • Considerable knowledge of federal and state regulatory rules and policies as they relate to service organizations.
  • Working knowledge of Medicare
  • Excellent organizational skills

  • Ability to coordinate and facilitate meetings
  • Ability to work as a team member by always bringing issues, concerns, or pending actions to the attention of all other COS members so they can be discussed and resolved between team members.

  • Ability to communicate professionally in multiple written and verbal formats (i.e. emails, required documentation, phone calls, etc.)

  • Ability to develop, nurture, and respect self-advocates and family member advocates.
  • Familiarity with Tennessee’s Department of Intellectual and Developmental Disabilities services a plus and the Maps Program.
  • Familiarity with Microsoft Word documents and templates is a plus.
  • Strong typing skills are a plus.
  • Ability to be a team player/member.

Benefits

  • The Maps Coordinator will have the option of working from home.
  • The Maps Coordinator will have the option of flexing his or her work hours on a day-to-day basis, depending on the need at the time. Work hours will consist of up to 20 hours a week with the goal of building the program into a full-time position. Start and end times will vary based on meeting schedules, visits, training responsibilities, etc.

$700.00 per month toward the health plan

Vacation and Sick time earned monthly (Two weeks vacation accumulate annually)

Retirement contribution after one year of employment
