Catholic Charities is seeking candidates for the position of Referral Coordinator to work in our Pregnancy, Parenting, and Adoptions program to assist with a new grant we have received from the State of Tennessee to work with pregnant women and families with children under 24 months old. This is a four-day-a-week position with benefits and based in Davidson County. Agency hours are Monday through Friday, between 8:00 am to 4:30 pm. The office location is 2806 McGavock Pike in the Opryland area of Nashville.

The primary responsibilities for this Long-term Case Manager position are to provide case management services to those clients who participate in the Pregnancy, Parenting, and Adoptions program for 3-6 months to reduce maternal health outcomes and increase infant mortality rates and health benefits for children under 24 months. This position will play a key role in helping mothers-to-be and young families access needed wrap-around services to stabilize or enhance their opportunities to be self-reliant and take their families to the next level. 

Job Responsibilities

  • Provide long-term case management services to participants in the program for 3-6 months.
  • Provide clients with the necessary social service assistance, and in-house referrals and have knowledge of external resources to appropriate agencies for services.
  • Conduct workshops/groups on identified topics to improve life conditions for families in the program.
  • Assist clients in scheduling appointments to access services.
  • Assist clients with developing and maintaining a Family plan to reach their goals.
  • Monitor client progress and growth and ensure full wrap-around services are available to help clients in reaching their goals. 
  • Attend community events to promote program awareness and growth.
  • Participate in outreach and recruitment activities to ensure that the program reaches its goals. 
  • Maintains complete and timely notes and other required documentation required by the grant (assessments, progress notes, quarterly reports, service plans, closing summaries, etc.)  according to Agency guidelines, using the database provided by the agency and within Agency time frame. 
  • Ability to work from other satellite offices and perform community fieldwork if necessary.
  • Some experience and understanding of trauma and Trauma Informed Care practices. 
  • Assist other team members on an as-needed basis and be able to work as part of a team.
  • Actively participates in supervision sessions, periodic team meetings, and training.
  • Performs other duties as assigned. 

Qualifications

Education/Experience:

  • Bachelor’s degree in social work or related field with experience in working with diverse communities.
  • 2 years experience working in social service or case management. 
  • Basic casework skills are relative to the needs of families experiencing multiple problem areas such as housing, financial stability, and short-term crisis situations.
  • Knowledge of basic group facilitation skills. 
  • Must possess a valid driver’s license and have reliable transportation.

Other Skills or Requirements:

  • Effective interpersonal skills.
  • Strong verbal and written communication skills.
  • Ability to work with individuals experiencing a crisis.
  • Computer proficiency with Microsoft Office applications; ability to learn databases.
  • Knowledge of community or other available resources that serve the needs of families who are experiencing multiple problem areas.
  • Ability to exercise good judgment, courtesy, and tact in dealing with the public and staff.
  • Ability to handle confidential information with extreme professionalism.
  • Ability to plan, organize, and work well under pressure and/or meet deadlines.
  • Schedule flexibility to accommodate the needs of the client.
  • Ability to follow tasks through to completion.
  • Ability to work well as a team and independently with minimal direction.
