Job Summary

We are seeking a qualified Case Manager/ Facilitator. This position provides intensive case management services in an Outpatient treatment setting. This individual must have knowledge and experience providing Addiction Severity Index (ASI) and ASAM placement criteria to determine client’s level of care and need for continued treatmment. Coordinates all necessary recovery and mental health care, along with associated supportive services for clients in community. Develops treatment plans and treatment plan updates in collaboration with other treatment team members. Facilitates psychotherapy and/or psycho-educational groups. Knowledgeable of evidence- based curriculum. Must be able to work Monday – Thursday, 11:00 am – 9:00 pm.

Job Responsibilities

  • Provide case management services to Intensive Outpatient treatment clients.
  • Provide ASI/ASAM assessment and placement as needed to determine client’s level of care.
  • Develops treatment plans and treatment plan updates in collaboration with other treatment team members.
  • Ensure timely and professional documentation to support each client interaction and coordination of services
  • Coordinating all necessary recovery and mental health care, along with associated supportive services for clients in community.
  • Connect clients with community resources and sober living environments.
  • Monitoring medication compliance and use of co-occurring services in community
  • Facilitate psychotherapy and/or psycho-educational groups Monday – Thursday.
  • Knowledgeable of evidence- based curriculum, i.e., Seeking Safety, MRT, Helping Women/Men Recover, Relapse Prevention, etc.
  • Contribute to treatment team to assess client progress and concerns
  • Create or provide weekly progress reports for Recovery Court staffing.
  • Attend Recovery Court staffing weekly
  • Complete intake orientation with all clients admitted to Intensive Outpatient programming as needed.
  • Primary liaison between probation and Therapeutic/ Recovery Court to communicate and facilitate client engagement and retention in   clinical services.
  • Conduct random alcohol and drug screening of clients (as needed)
  • Create and maintain resource list for clients in order to have proper resources for community connections.

Qualifications

Preferred Qualifications:

CPRS or LADAC. A minimum of 3 years experience providing case management, working with substance-related recovery needs and pursuing LADAC. Bachelor’s degree.

Must be knowlegeable of the Addiction Severity Index (ASI) assessment and ASAM.
