Job Summary

MAJOR FUNCTIONS:

This individual serves under the general supervision of the Executive Director. This individual is responsible for coordinating the agency’s efforts in continually enhancing the quality and consistency of our adult program operations.

Job Responsibilities

MAJOR RESPONSIBILITIES:

  • Participate in the agency’s Person-Centered Planning process as a PCP Team member.
  • Oversee the application of THERAP documentation, liaison with THERAP representatives, and trainer for Therap applications as part of onboarding new employees as needed.
  • Assist with reviewing and interpreting written responses from the monitoring entity to the agency with regard to contract compliance. Provide written responses to the Tennessee Department for Intellectual and Developmental Disabilities (TNDIDD), TennCare Utilization Reviews, and Licensure as needed.
  • Collect and evaluate data on unannounced site visits, medication variances, falls, reportable incidents, investigations, and emergency drills. Identify and report any potential trends in these areas to the Director of Adult Services and Executive Director.
  • Conduct a minimum of one chart audit per month of enrolled adult services participants.
  • Participate in all training deemed necessary to perform duties and responsibilities effectively. Keep required training up-to-date.
  • Coordinate data entry of Service Plans, Individual Support Plans (ISPs), cost plans, and other quality-related data with Program Managers.
  • Serve as backup to the Incident Management Coordinator and serve on the incident management committee.
  • Monitor THERAP contact notes, attendance, and billing.
  • Conduct and compile annual satisfaction surveys of family, staff, and individuals supported.
  • Assist with the preparation and response of the Quality Assurance Survey, Licensure, and the Financial Accountability Review (FAR) audit as needed. Be accessible and onsite during the annual State of TN QA survey (typically 4 successive days).
  • Report on service recipient demographics as needed for grant proposals and surveys.

Qualifications

  • Knowledgeable about the service system for individuals with developmental disabilities in Tennessee or present an ability to learn it
  • Must have above-average verbal and writing skills and excellent communication skills with professional staff outside the agency.
  • Experience in working with individuals with disabilities.
  • Well-organized and detail-oriented.
  • Knowledge of computer skills, including Word, Excel, and databases, is required.
  • Must possess and maintain a valid Tennessee driver’s license.
  • Must possess flexibility in scheduling work hours.
