Job Summary

The Coach works one-on-one with a participant in a collaborative process to help identify and achieve goals. Using Family-Centered Coaching, a Coach strives to serve the whole family and is aware that families are the experts on their own strengths and perceived challenges. Coaches are just one member of the CARE team that includes the Employment Navigator, Financial Counselor, Engagement Coordinator (EC), and other MOBC and partner support systems. The Coach is often the primary contact for the family and is responsible for enrollment of participants into the Family Empowerment Program (FEP), obtaining all necessary signed documents, assessing family for needs, guiding families in goal setting around core areas of service, referring family to needed partners or providers in the community, and updating progress in the shared database until family exits the program. This coaching position works from the Fannie Battle Day Home in east Nashville.

Job Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES                                                                                                                                                       

  • Maintains 25 families enrolled in a program at a given time, with a goal of serving 30 families per year.
  • Provides holistic and comprehensive services to all participants including intake assessments, goal setting, long-term action plan development, progress monitoring, budgeting, advocacy, and referrals.
  • Meets with family on a regular basis for 6-18 months on average. During the first 3 months, a majority of follow-up meetings should be held in person rather than over the telephone.
  • Connects families with needed and available community resources. Follow up with participants and agencies as appropriate to document the use/success of the referral.
  • Inputs accurate and complete data for all interactions with participants into the agency database.
  • Ensures that all documents submitted on behalf of a participant are valid.
  • Communicates regularly with Employment Navigator, Financial Counselor, and other support systems as needed. Participates in regular staff meetings, staff training programs, and supervisory sessions, and accepts responsibility for developing a cohesive and positive team culture.
  • Assists in maintaining meaningful relationships with partnering organizations including landlords, churches, employers, volunteers, and local businesses.
  • Leads and organizes monthly group time for participants after center hours.
  • Actively makes efforts to seek out and meet with other local community resources partners that could informally collaborate with the FEP to provide additional services and support to those in FEP.
  • Collaborates with the CARE team to ensure that any children in the FEP who are nearing the age of “phasing out” of their Early Learning Center (ELC) are connected and linked to services at their local Family Resource Center (FRC).
  • Adheres to Agency policies, procedures, and professional code of ethics.
  • Performs other duties as assigned. 

Qualifications

MINIMUM QUALIFICATIONS

Education and Experience:

  • Bachelor’s Degree in Social Work or a related field.
  • One or more years of experience working with persons experiencing poverty

  • Possess a valid driver’s license. Ability to legally operate a motor vehicle and provide own transportation.
  • Ability to speak, read, and write in English.
  • Computer literate including familiarity with word processing, spreadsheet, database, desktop publishing software, email, and the internet. Preferred skills in Microsoft Word, Excel, and Outlook outside of the Agency and organizing pertinent data and program materials.
  • Excellent verbal and written communication skills.
  • Ability to effectively communicate through oral, written, and technological means; actively listening and expressing oneself clearly, to ensure those that need specific information acquire it; plan and deliver communication that makes an impact and persuades the audience
  •  Regular work attendance.
  • Relationship Management: builds relationships internally and externally through communication efforts with the CARE team, the FEP team, FEP partners, potential FEP partners, and the community. Ability to cultivate, build, maintain, and strengthen partnerships with a diverse group of people, including low-income, urban residents.
