Job Summary

 

The Village at Glencliff is a new 501(c)(3) organization with a mission to provide dignified, loving, and hospitable medical respite to our friends experiencing homelessness here in Nashville, TN. We offer a comprehensive array of services to our residents, who stay with us from hospital discharge until permanently housed. We will be working very closely with the Nashville Continuum of Care (CoC) to provide services to our residents.

The role of the Care Coordinator is to utilize agency and community resources to offer residents every opportunity to break cycles of poverty and unlock pathways to permanent housing, healing, and hope. The Care Coordinator works with residents on their Individualized Service Plans, assists residents through the application process for available resources, and addresses resident needs regarding transportation, employment, housing, and medical/mental health/recovery issues. The Care Coordinator ensures that all residents are adequately oriented and accountable to the program rules and guidelines and is responsible for the management and implementation of the Program Handbook. This position also coordinates with other team members to maintain a mission-focused environment in which residents receive the highest quality of compassionate and dignified care while concurrently utilizing evidence-based practices, such as Trauma-Informed Care. As the residents’ most direct staff contact, the Care Coordinator interacts with residents in a way that fosters health, mutual respect, clear communication, and positive self-regard.

Job Responsibilities

 

  • Coordinate regular meetings with each resident, including but not limited to ongoing assessment and goal-setting, referrals to resources, directing and supporting as needed to help residents achieve self-sufficiency, and crisis intervention.
  • Increasing residents’ awareness of available services applicable to individual needs while also assisting them in accessing those resources in a way that encourages personal responsibility and self-sufficiency.
  • Responding to all referrals within two business days and completing a Screening Form with eligible people who have been referred.
  • Scheduling orientation and move-in days for new residents and orienting new residents to The Village, completing all necessary intake paperwork, and reviewing the Resident Handbook with them.
  • Ensuring that emergency numbers are listed in each home when the resident moves in.
  • Creating and maintaining a file for each resident with appropriate and necessary paperwork and documentation.
  • Meeting with each resident to fill out an Individualized Service Plan (ISP) within the first seven (7) days of their residence. For COVID-19 Isolation residents, the ISP will be more limited in scope but equally important and will be completed within two (2) business days of the resident moving into The Village at Glencliff.
  • Ensuring that each Resident is, upon receipt of consent, documented in HMIS.
  • Following and enforcing all Program Rules and Guidelines onsite with the support of other program staff. If problems arise or special circumstances need to be observed, discussing the matter with the Executive Director.
  • Issuing Conduct Warnings and creating Behavior Contracts when rules are broken or behavior issues arise.
  • Filing Grievance Forms in a Grievance Log.
  • Issuing Late Passes when deemed appropriate.
  • Assisting the resident in filling out an Extended Absence Form Agreement when appropriate.
  • Doing regular house inspections to ensure that the micro homes are clean and that no unreported maintenance issues are going on.
  • Working with other team members to assist in the identification and completion of planned or needed replacements, repairs, and renovations of building, furniture and/or appliances for The Village property.
  • Participating in agency meetings and events and collecting appropriate data for residents at The Village.
  • Filling out an Exit Survey with residents when they move into permanent housing.
  • Participating in the design, delivery and/or facilitation of group therapeutic, educational, support and/or recreational activities.
  • Other duties as assigned.

Qualifications

 

  • Strong preference for candidates with demonstrated experience with homeless outreach, transitional housing, addictions, and mental health services, including Nashville HMIS.
  • Knowledge of Nashville’s unhoused community and how to navigate existing resources.
  • Commitment to the mission and values of the Village at Glencliff.
  • Commitment to fostering professional and supportive working relationships with colleagues, residents, and volunteers.
  • First Aid and CPR certifications, or the willingness to become certified.
  • Valid documentation of full dose of COVID-19 vaccination.
  • A means of transportation and a valid Tennessee driver’s license.
    Lived experience of homelessness considered an asset.

  • Effective written, verbal, and interpersonal communication skills.
  • Computer literacy (Microsoft Word, Office, Excel, and Internet).
  • Demonstrated ability to manage crisis situations compassionately and effectively.
  • Demonstrated ability to work with diverse groups of people.
  • Comfortable in individual, group, and public speaking environments.

Additional Information

Benefits

Salary plus an additional stipend equal to 20% of annual salary as an allowance for health insurance. If insured, The Village at Glencliff will pay employees a yearly lump sum of $2,000 in lieu of paying for health insurance.  The Village at Glencliff recognizes 15 holidays per calendar year, and all employees receive four (4) weeks of Personal Time Off annually.
