Job Summary

Skills & Knowledge:

  • Willingness to partner with a Family Support Specialist (FSS) in order to provide a team-based approach to in-home services.
  • Willingness to be trained in (TARGET) Trauma Affect Regulation: Guide for Education and Therapy
  • Willingness to be trained in mental health trainings such at MHT-JJ (Mental Health Training-Juvenile Justice)
  • Knowledge of children’s mental, emotional and social development and well-being.
  • Knowledge of and experience working with juvenile justice, educational, and other child-serving systems.
  • Knowledge of how to develop family-driven and strength-based treatment/behavior plans with families.
  • Knowledge of system of care/wraparound principles/practices.
  • Excellent organizational, written, communication and problem-solving/creative thinking skills.
  • Excellent collaboration, customer service, and leadership skills.

Job Responsibilities

Duties & Responsibilities/Principal Activities:

  • Completes Strengths-Based Assessments (CANS, ACES, CGSQ at intake and discharge,) Interpretive Summary, and Individual Service Plan in collaboration with the youth, family, and Family Support Specialist (FSS) to identify strengths and needs.
  • Works as a team with the Family Support Specialist to ensure coordinated service delivery and at least weekly face-to-face contact with family/youth.
  • Attends court/CFTMs and communicates with Youth Service Officers regarding youth/families’ participation in services and progress in completing goals established to address issues contributing to court involvement/delinquent charges.
  • Assist FSS in identifying community resources and helping family engage with resources that will sustain family’s progress beyond discharge from JJR program
  • Available by cell phone 24/7 for crisis support and will create a crisis support plan.
  • Availability to work outside of the general operating hours to meet the needs of the family/youth.
  • Documents progress, safety plan, treatment plan, etc. and enter into the EHR.
  • Complete transition plan/discharge summary.
  • Facilitate MHT-JJ Training for judicial staff, law enforcement staff, partners, and members of the community
  • Ensure client information is secured consistent with HIPAA requirements.
  • Ensure that we conform to CARF standards applicable to the services provided.
  • Other duties and/or tasks as assigned.
  • Must have a valid driver’s license and proof of current auto insurance.
  • This position covers Robertson County & Sumner County.

Qualifications

Master’s Degree in counseling/therapy or closely related field is required. Preference given to parents/caregivers or family members of children with SED. Non-profit organization experience and work history within child serving systems, (especially Juvenile Justice) also strongly preferred.
