STATEMENT OF THE JOB

The CDMS Program Specialist is responsible for supporting the CDMS Program Coordinator in all aspects of CDMS work ensuring all aspects of the TN Center for Decision-Making Support are implemented in accordance with the work plan/s and scope of services. This includes but is not limited to the development and implementation of the strategic plan, creation of online and printed resources, facilitation of the Tennessee Center for Decision-Making Support Advisory Group, and collaboration with other organizations.

Job Responsibilities

Specific Job Duties

  • Support Program Coordinator with the implementation of the scope of services
  • Assist with the development and implementation of the strategic plan for creating the Tennessee Center for Decision-Making Support
  • Assist with CDMS website maintenance and updates
  • Identify and develop opportunities to increase Tennessee Center for Decision-Making Support services and support.

Legal and Content Knowledge Expert

  • Develop a working knowledge of TN statutes related to decision-making supports
  • Become and maintain LifeCourse Nexus Ambassador status. Complete LifeCourse badge and training modules (badge and certification/s)
  •  Assist with the development of accessible Tennessee-specific resources for individuals and families about the various types of decision-making supports (which may include handouts, videos, webinars, etc.)
  • Serve as a state-wide resource on a variety of decision-making supports
  • Ability to understand legal ease related to decision-making documents and statutes
  • The ability to create plain language content related to decision-making supports

 Service Provision

  • Provide information to individuals and families related to decision-making support for vulnerable adults
  • Communicate with a variety of individuals about Decision-Making Support options, including self-advocates, families, agency personnel, educators, attorneys, judges, and more.
  • Provide limited one-on-one assistance to include decision-making support consultations, supported decision-making coaching, transition services, future planning, etc.
  • Conduct complex case assistance regarding decision-making support options for MCOs, providers, agency personnel, and other professionals.
  • Provide oversight support assistance for legal cases related to conservatorship modifications and rights restoration. 

Outreach, Collaboration, and Marketing

  • Assist with the development of a network of attorneys knowledgeable about the various options for decision-making support and who are willing to support individuals in the reversal of conservatorships
  • Develop and maintain a legal database on the Center for Decision-Making Support website.
  • Network with and collaborate with other organizations and state departments including, but not limited to, Managed Care Organizations, service providers, and post-secondary programs for people with intellectual and developmental disabilities to share information about decision-making support
  • Create training presentations on different Decision-Making Support options, transition services, and future planning.
  • Present at conferences on different Decision-Making Support options
  • Collaborate with stakeholders to identify service gaps and resolutions
  • Possess strong technical writing skills for grant proposals, policies, procedures, video scripts, websites, and resource content.
  • Keep current on any in-service requirements for The Arc Tennessee
  • Support other activities of The Arc Tennessee such as family meetings, board committees, and conferences
  • Other duties as assigned

Reporting

  • Complete monthly work plan
  • Complete timesheets and expense reports
  • Complete all other reports as required

Qualifications

  • Minimum one year experience in human services, case management, and working with individuals with disabilities
  • Ability to work independently on a variety of activities
  • Excellent written and verbal communication skills
  • Ability to work collaboratively with others
  • Computer literate using Microsoft Office Suite Software
  • Reliable transportation
  • Bi-lingual a plus
