Job Summary

Provide clinical services to a population of children and adolescents. Provide intervention, assessment/intake, treatment planning, triage, individual therapy, group therapy, and/or play therapy, as necessary. Clinicians will ensure that all documentation necessary to meet contract, licensure, and appropriate behavioral health organization requirements pertaining to these services will be completed in a timely manner per VBHCS standards. Employees are expected to meet the productivity standards as outlined by VBHCS, based on the number of hours worked per month. 

Provide care management to adults and children focusing on strengths of individuals and families. Care management services assist individuals in gaining access to needed medical, social, educational, and supportive services. Care management is primarily a home and community-based service designed to be conducted within natural settings. A member of the care management team should be available 24/7. 

Employee is responsible for completing all documentation regarding client services in accordance with VBHCS P&P QM-GEN-001 – Signing of Clinical Documentation; attending staff meetings and supervision sessions as required; completing required training as assigned and/or scheduled; meeting the productivity billable unit standards as assigned by VBHCS; and monitoring their billable services report.

Job Responsibilities

Counselor Duties include, but are not limited to:

Clinicians will ensure that all documentation necessary to meet contract, licensure, and appropriate behavioral health organization requirements pertaining to these services will be completed in a timely manner per VBHCS standards. Employees are expected to meet the productivity standards as outlined by VBHCS, based on the number of hours worked per month.

Clinical services will include individual therapy, intakes, assessments, such as DLA 20,Columbia Suicide Assessment and PHQ9, discharge planning, treatment planning, appropriate referral to internal/external services and support systems, and staffing of consumers. Treatment population will be adults, children and youth and their family system.

Liaison and/or consultation with other service providers and referral sources necessary to achieve treatment goals will be required. This may include; court, schools, foster homes, relatives, care managers, and/or state agencies.

OTHER DUTIES:

  • Function in the school setting as a team member with the teachers and educational assistants.
  • Extensive liaison works with families and the community.
  • Provide consultation and treatment planning with teachers of students being served. This will include involvement in IEP Team meetings to assist in the development and implementation of goals and objectives of the IEP.
  • Conduct liaison activities between the school system, parents, courts, and other community agencies.
  • Provide home visits to families of students being served under this contract.
  • Documentation of services provided.
  • Exchange information as needed with VBHCS staff.
  • Exchange information when appropriate with classroom teachers, county behavior specialists, court system, and other community agencies involved with clients.

Other duties as assigned. 

Care Manager Specific Duties:

  1. Comprehensive Care Management – Initiate, complete, update, and monitor the progress of a comprehensive person-centered care plan (as needed).
  2. Care Coordination – Participate in the patient’s physical health treatment plan as developed by their primary care provider as necessary. Support scheduling and reduce barriers to adherence for medical and behavioral health appointments. Proactive outreach and follow up with primary care and behavioral health providers.
  3. Referral to Social Supports – Identify and facilitate access to community supports (food, shelter, clothing, employment, legal, entitlements and all other resources). Communicate patient needs to community partners. Provide information and assistance in accessing services.
  4. Patient and Family Support – Provide high-touch in-person support to ensure treatment and medication adherence. Provide caregiver counseling and training. Identify resources to assist individuals and family supporters.
  5. Transitional Care – Provide additional high touch support in crisis situations. Participate in the development of discharge plans for each hospitalization. Develop a systemic protocol to assure timely access to follow-up care post discharge. Establish relationships with other treatment settings. Communicate and provide education.
  6. Health Promotion – Education the patient and his/her family on independent living skills with attainable increasingly aspirational goals.

Reports/Record Keeping:

  1. Complete all progress notes, service plans, memos, and related client service documents in accordance with VBHCS P&P QM-GEN-001 – Signing of Clinical Documentation..
  2. Complete and forward to supervisor all paperwork as scheduled (time sheets, service plans, client contact logs, etc).
  3. Complete and turn in all mileage reimbursement claims (when appropriate) to the supervisor at least monthly.
  4. Review electronic tracking report “My Billable Services,” and report any errors that need corrections to the Chief Information and Customer Service Officer by the 3rd work day of the following month.

Consistently treat consumers with respect as individuals

  1. Connect in a positive manner with consumers/clients and family members.
  2. Solicit consumer/client opinions and input.
  3. Listen and respond with respect.

Work within team environment

  1. Actively participate in the team process, and utilize team resources. Meet with the team on scheduled staff meetings and discuss any concerns regarding working with consumers, etc.
  2. Employ good communication skills (listening, reflecting, role model, etc.)
  3. Report and share information promptly with the doctor, team, and others, as appropriate.

Clinical Services

Provide emergency services for clients in crisis during office hours and after hours as assigned.

Provide the following care management services for assigned clients:

  1. Assessment of mental health, psychosocial and physical needs.
  2. Referral to internal/external services and support systems.
  3. Consultation with other service providers and referral services.
  4. Preparation of reports for agencies, courts, other service providers, etc.

Maintain clinical records according to VBHCS Policy & Procedure and Quality Assurance standards.

Participate in staffing, administrative staff meetings, supervisory sessions and other Center committees as assigned.

One hour of Corporate Compliance Training and four hours of Risk Area Training annually.

Qualifications

Must have a Master’s Degree in the Behavioral Sciences (social work, counseling, or psychology). Experience in group, family, and individual therapy preferred. Possess excellent communication and networking skills.
