Job Summary

To provide case management and other support services to members of the Supported Housing program toward the goal of more independent community integration.

Job Responsibilities

Support services for members.

  • Refer and link members to services including primary care and other healthcare providers, case management, educational programs, and other services identified by the member.
  • Communicate pertinent information and plans to the primary case manager. 
  • Transport clients to appointments when the primary case manager and other supports are unable to do so.
  • Assist members with obtaining vital records as needed. 
  • Assist the Employment Team with members as needed.
  • Assist with transition/discharge planning.
  • Assist members with obtaining hygiene products and clothing upon move-in as needed.
  • Assist with WRAP, IMR, and additional skills training groups as needed.
  • Participate in at least two significant direct-care interactions with each member per month.

Develop Recovery Plans

  • Complete the initial Recovery Plan with members within 14 days of admission. 
  • Ensure that the Recovery Plans are appropriately updated and modified every 30 days to meet the ongoing needs of the members. Place the updated Recovery Plan in the member binder located at each Supported home.
  • Ensure that the Recovery Plan is implemented and that interventions are carried out consistently and appropriately.
  • Teach, demonstrate, and actively assist members in developing the skills to maintain stability in the community.

Chart Documentation

  • Ensure that each member is scheduled for a physical examination within 30 days of entering the program and document it in the chart.  Ensure that each member receives a physical exam at least once per year thereafter.
  • Complete intake documentation upon admission including scheduling a physical exam, assisting in identifying PCP, or requesting documentation from recent (within 90 days) exams, CSSR-S, and bed bug treatment.
  • Complete and submit SNAP benefits application for each member upon move-in.
  • Write Progress Notes in EHR for each significant contact with members within three business days of the encounter.
  • Read through member progress notes as needed to gain familiarity with member progress and barriers.
  • Complete DLAs every 90 days for each member. 
  • Submit concurrent reviews every 90 days and discharge information to TennCare MCOs. 
  • Document discharges in EHR.

Medication Documentation

  • Work with Supported Housing Supervisor, Recovery Coaches, and Support Coordinators to ensure each member has a sufficient supply of medications by responding to any refill needs, coaching members in building skills to obtain medications, and communicating with members’ medication providers and primary case managers.

Record keeping/Administrative Duties: 

  • Clock in and out using the electronic system. 
  • Records nightly absences in EHR weekly.
  • Complete standing orders for TennCare transportation as needed.
  • Assist Supported Housing Supervisor with Recovery Coach Meetings weekly.

Public/Staff Relations

  • Utilize voice and email. 
  • Communicate regularly with other staff to meet the needs of members and residents. 
  • Develop appropriate working relationships with other mental health, social services, and rehabilitation agencies.
  • Communicate with other mental health, social services, and rehabilitation agencies regarding members as needed. 
  • Communicate with members’ unpaid support as needed.
  • Work as a team player with other staff. 

Other Duties as a member of the Park Center Housing staff 

  • Participate in weekly supervision meetings with the Supported Housing Supervisor.
  • Assist with housing events.
  • Participate in on-call rotation.
  • Has good attendance in the program and informs supervisor in advance of time off.             
  • Follow all Personnel Policies. 
  • Other duties as assigned.

Qualifications

  • Bachelor’s Degree from an accredited college in social work, sociology, or other related field OR at least 2 years experience in a mental health or social service setting. 
  • Ability to work independently and with a team. 
  • Demonstrate exceptional problem-solving skills.
  • Ability to meet deadlines.
  • Good communication skills. 
  • Strong professional and ethical behaviors. 
  • Psychosocial rehabilitation or housing experience preferred. 
  • Must be able to show proof of personal automobile insurance.
  • Must be insurable under Park Center’s automobile insurance.
