Job Summary
Assisting families with navigating Katie Beckett Part B Waiver Services, foster families to navigate Tennessee Strong Families HRA benefit, and working with families through the Family Support Program.
Job Responsibilities
The Support Coordinator will be responsible for:
- Katie Beckett Part B (Children with Intellectual/Developmental Disabilities and/or high medical needs)
- Case Management Duties and Processes for children including
- Initial contact upon enrollment
- Service Initiation
- Ongoing phone contacts monthly, two face-to-face visits per year
- Yearly reassessment of the Level of Care
- Yearly planning meetings and annual updates to the Person-Centered Support Plan (PCSP)
- Assist families with accessing Healthcare Reimbursement Account (HRA) Benefits through Inspira
- Maintain records within the Therap system and Dropbox
- All families are in Williamson County
- For Tennessee Strong Families cases (Children with Intellectual/Developmental Disabilities who are in DCS Custody)
- Eligibility determinations
- Developing a PCSP outlining a general description of the child’s needs and updating it annually.
- Maintain records in Sharepoint and Dropbox
- Maintain contact with the foster family as set forth by the Department of Disability and Aging (currently one phone contact per month and two face-to-face visits per year).
- Working with the DCS Case manager and attending Child and Family Team meetings to ensure service goals are aligned and to review the status of services/supports paid through the HRA Account.
- Assist the foster family in resolving issues related to the use of the HRA debit card and reimbursement claims, including direct contact with the HRA vendor as warranted.
- Two face-to-face visits per year. Most live in Williamson County. There are a few in Rutherford and Dickson but any new cases will be in Williamson County only.
- Family Support
- Identify eligible families and with their participation, determine their needs and preferences
- Develop a written plan for the delivery and payment of services
- Maintain a waiting list spreadsheet of contacts
- Work with families to get documentation for reimbursement
- Review invoices within 7 days of receipt for completeness and reimburse by direct deposit or check
- Maintain the Family Support Quickbooks account
- Maintain the Family Support Reimbursement Spreadsheet and submit claims monthly
- Maintain records per DDA contract
- Maintain local Family Support Council (quarterly meetings)
- Assist with service coordination for all families
- Prepare records for the annual FAR Review
- Complete proposal for Family Support Program every 3 years
- All families are in Williamson County.
Qualifications
The ideal candidate will have a strong background in service coordination, excellent verbal and written communication skills, and a working knowledge of Microsoft Office and relevant database systems (therapy preferred). A bachelor’s degree in a human services field is preferred, though relevant experience may be considered in lieu of formal education. Reliable transportation with valid insurance is required.
This position requires flexibility, professionalism, and a commitment to person-centered practices. While the role is primarily office-based, there is flexibility for remote work after the training and probation period, along with occasional travel for meetings, visits, and training sessions.
- Bachelor’s Degree and two years of relevant experience.
- MUST be self-motivated.
- Familiarity with QuickBooks is a huge plus.
- Any Spanish Language is a huge plus.
