Job Summary
The Support Coordinator plays a critical role in assisting individuals with intellectual and developmental disabilities (I/DD) by ensuring they receive appropriate services and support to enhance their quality of life. This position is responsible for managing a caseload of up to 40 individuals, coordinating services, advocating for individuals’ rights, and ensuring compliance with all relevant policies and regulations.
Job Responsibilities
Key responsibilities include maintaining effective communication with Circles of Support (COS), providing families with information on HCBS, TennCare, ECF, and TN Family Strong Program services, and advocating for individuals’ inclusion and decision-making. The Support Coordinator is also responsible for collecting accurate data to develop person-centered service plans (ISP/PCSP), submitting documentation in compliance with DIDD policies, and addressing quality review findings in a timely manner.
The role requires strong organizational and time management skills, critical thinking abilities, and proficiency in conflict resolution. Additionally, the Support Coordinator must conduct monthly visits with individuals, assess potential risks, and report any incidents in accordance with state guidelines. They are also expected to provide training and guidance to team members, assist families in maintaining Medicaid eligibility, and facilitate appeals as necessary.
This position requires flexibility, professionalism, and a commitment to person-centered practices. While the role is primarily office-based, there is flexibility for remote work after the training and probation period, along with occasional travel for meetings, visits, and training sessions.
For those assigned to the Katie Beckett Program (Part B) and TN Family Strong Program, additional responsibilities include monitoring service implementation and quality, ensuring adherence to budget guidelines, coordinating eligibility redeterminations, and addressing service issues within 30 days.
The ideal candidate will have a strong background in service coordination, excellent verbal and written communication skills, and a working knowledge of Microsoft Office and relevant database systems (therapy preferred). A bachelor’s degree in a human services field is preferred, though relevant experience may be considered in lieu of formal education. Reliable transportation with valid insurance is required.
This role is essential in ensuring that individuals with I/DD receive the support they need to lead self-determined, meaningful lives within their communities.
Qualifications
Education Requirements:
- Bachelor’s degree in a human services field; OR
- Bachelor’s degree in an unrelated field with at least one year of relevant experience; OR
- Associate’s degree with two or more years of relevant experience; OR
- High school diploma/GED with four or more years of relevant experience.
- Strong organizational and time management skills.
- Ability to facilitate and coordinate meetings effectively.
- Critical thinking and strategic planning abilities.
- Excellent verbal and written communication skills.
- Proficiency in conflict resolution.
- Familiarity with Microsoft Office applications (Word, Excel) and database systems (Therapy preferred).
- Knowledge of Tennessee DIDD services is a plus.
- Reliable transportation with valid insurance is required.
Essential Functions
Coordination and Advocacy
- Maintain effective communication with Circles of Support (COS).

- Advocate for individuals’ rights, inclusion, and decision-making.
Planning and Documentation
- Collect accurate and timely data to develop person-centered service plans (ISP/PCSP).
- Ensure all documentation complies with DIDD policies and agency standards.
- Submit service amendments and address quality review findings within required deadlines.
Monitoring and Reporting
- Conduct monthly visits, with at least one visit every three months at the individual’s home.
- Assess risks and ensure individuals’ safety by communicating concerns to stakeholders.
- Report incidents of abuse, neglect, or emergencies in accordance with state guidelines.
Training and Support
- Provide guidance to team members and assist in training new staff.
- Support families in maintaining Medicaid eligibility and navigating appeals.
Work Expectations
- Maintain punctuality and consistent attendance.
- Complete monthly notes and billing on time (e.g., by the 30th/31st).
- Adapt to changes in workload and scheduling as needed.
- Uphold confidentiality and professionalism in all interactions.
- Manage a full caseload of up to 40 individuals.
Work Environment
