Job Summary

Murfreesboro Cold Patrol’s Street Outreach and Housing Retention Specialist will be responsible for establishing rapport and building relationships with individuals experiencing homelessness and serving as a primary point of contact for both those experiencing homelessness and community partners for outreach and housing sustainability. This individual will collaborate with local emergency response teams, service providers, volunteer teams, and other organizations to work in the best interest of those experiencing homelessness as they access needed resources and support. The ideal candidate for this position will also use creative and collaborative techniques for rapid rehousing and offer comprehensive case management to minimize the impact of homelessness on both individuals and families. They will strategically work with community partners and MCP volunteers to end the revolving door of homelessness through outreach, advocacy, and resource support.

Job Responsibilities

  • Visiting local encampments, building rapport, and maintaining relationships with individuals experiencing homelessness throughout the week
  • Making contact and building relationships with individuals and families experiencing homelessness while providing a quick turnaround on resources, warm handoff to referrals, and comprehensive outreach support to move individuals from the streets to a sustainable next step
  • Creatively networking to find opportunities for housing and other critical services in an environment with limited resources.
  • Provide comprehensive case management, treatment planning, and oversight for all clients (quickly and effectively overcoming obstacles, accessing critical resources, goal setting, helping establish a support system, etc.)
  • Assisting clients as they navigate application processes, obtain and provide necessary documentation for financial assistance and housing; evaluate applicant information against program eligibility criteria
  • Helping eligible clients apply for and obtain social services (food stamps, disability, phone, internet, etc) and other resources as needed and applicable
  • Maintaining consistent communication with clients both by phone and in person, ensuring clients feel capable and have the tools they need to succeed
  • Attending weekly outreach and community meetings to effectively advocate for those we serve and encourage strong, positive communication between outreach volunteers and community partners
  • Keep up-to-date case notes, track donations, and complete all monthly tracking and grant requirements in a timely manner
  • Ensuring clients do not fall through the cracks while making external referrals, providing warm hand-offs and follow-up services
  • Transporting individuals as needed
  • Telling the story of the organization in an ethical and strengths-based way through newsletters, blogs, and social media accounts
  • Other responsibilities as needed

Qualifications

  • Bachelor’s degree in related field AND two years of direct service with individuals experiencing homelessness and poverty OR any equivalent combination of experience/education from which comparable knowledge, skills and abilities have been gained
  • Demonstrated patience, kindness, and compassion for individuals who have experienced trauma and who may struggle with intensive mental and physical health needs, addiction, and behavioral health issues
  • Alignment with Murfreesboro Cold Patrol’s Mission Statement and Core Values
  • Valid driver’s license and insured vehicle
  • Trustworthiness and transparency
  • Experience in homeless outreach or homeless services and/or case management in a related field
  • Self-motivated and able to work independently while remaining organized and meeting deadlines
  • Demonstrated competency in working with community partners and volunteers
  • Knowledge of (or willingness to be trained in) evidence-based practices including Trauma-Informed Care, Strengths-based approaches, and Motivational Interviewing
  • Strong communication skills- written, verbal, and interpersonal communication
  • Proficiency in basic computer skills and data tracking and management
  • Detail-oriented
  • Ability to collaborate well with others and be a strong team player
  • Care and concern for the unity and well-being of Murfreesboro Cold Patrol
