MAJOR FUNCTIONS:

Under the general supervision of the Franklin Residential Program Manager, the person in this position is responsible for coordination, implementation, record keeping, and general oversight of all medical services provided to persons served by the Franklin Residential Division. This may include, but is not limited to: attending and documenting medical appointments, training staff on therapy and medical plans, upkeep of medical records according to DIDD requirements, ordering medications, processing MAR’s, and participating in agency, COS and ISP meetings. The person in this position will be included in the after-hours emergency on-call rotation for the Franklin Residential Division. The person in this position is expected to be knowledgeable of all DIDD guidelines and requirements related to the services performed.

Job Responsibilities

MAJOR RESPONSIBILITIES:

1. Follow WAVES mission to – Empower individuals with intellectual and developmental disabilities to progress toward their full potential.

2. General oversight of all medical appointments, medications, and medical-related services for all persons served in the Franklin Residential Program.

3. Maintain and file medical records according to DIDD requirements, including but not limited to, physician’s statements and consultation forms.

4. Accompany persons served to priority medical appointments and emergency appointments. Delegate and assign nonpriority medical appointments to House Managers and DSP’s.

5. Oversee Therap database as it relates to medical documentation, medical history updates, and medical appointments.

6. Oversee monthly autofill of medications and process medications upon delivery. Order new or individual medications as needed.

7. Coordinate the delivery of medications to each home.

8. Ensure that each home has monthly medications and MAR’s in a timely manner.

9. Review monthly MAR’s for med variances and re-train staff accordingly.

10. Manage expired/unused medications and return to the pharmacy in a timely manner.

11. Obtain consent for psychotropic medications from conservators.

12. Act as the primary contact for pharmacies and physicians.

13. Assist Program Manager with the oversight and management of persons served health benefits.

14. Train staff on medical diagnosis, medical plans, medical documentation, and record requirements.

15. Participate in the after-hours emergency on-call rotation for Franklin Residential Team.

16. Be familiar with DIDD regulations for Protection from Harm. Report and document all incidents in a timely manner.

17. Conduct and document unannounced on-site visits to each of the division locations, as delegated by Program Manager.

18. Provide medical updates to be included in monthly reviews and biweekly Incident Management Committee meetings.

19. Attend Circle of Support and ISP meetings as requested for persons served.

20. Attendance and participation in division meetings and regular communication with the Franklin Residential Program Manager and the Adult Services Director, regarding medical updates, progress, special needs, or obstacles which may affect quality services to service recipients.

21. Provide training to House Managers and Franklin Day Program staff as needed in meeting medical needs for persons served.

22. Be flexible in scheduling work hours each week.

23. Other duties and responsibilities may be assigned by the Program Manager or Adult Services Director to ensure the smooth operation of all homes in the Franklin Residential Division, and the agency as a whole.

24. Participate in the Train-the-Trainer classes and the agency training for new staff when requested.

25. Maintain all required endorsements and certifications.

 

 

Qualifications

JOB QUALIFICATIONS:

 

High School Diploma or GED. Two years of experience working as a Direct Support Professional with individuals who have intellectual and/or developmental disabilities, reflecting increasing levels of responsibility. High level of knowledge and experience with the Department of Intellectual and Developmental Disabilities requirements and policies pertaining to medication and medical oversight in supported living environments. Must be able to obtain CPR and First Aid certifications, and Medication Administering Training. Must possess flexibility in work schedule. Must possess and maintain a valid Tennessee driver’s license.
