Job Summary

The Resident Service Coordinator acts as a liaison between Urban Housing Solutions and local hospitals, clinics, and outside agencies. This role works with residents to improve income self-sufficiency to meet housing and financial stability goals. Duties include but are not limited to provide residents access to service, network with community partners, provide education and support service resource to residents, complete annual assessments and reassessments, make appropriate referrals for residents who require additional assistance; plan health related activities for residents, and serve as the lead on all other on-site resident programming (i.e., budgeting workshops, etc.) at respective properties. Job Responsibilities

• Act as a liaison between Urban Housing Solutions (UHS), other community health agencies, hospitals, and clinics maintaining professional communication
• Provide immediate follow-up to residents with medical issues (i.e., recent trip to ER, negative health screening results, chronic disease)
• Serve as the Lead on all health-related matters for respective properties
• Work to coordinate services for UHS most vulnerable, home bound residents to receive wrap-around and medication pick-up services to stabilize and maintain their housing
• Coordinate Service Animals/Reasonable Accommodations requests with the respective property manager staff
• Provide service coordinator duties to all eligible residents on your designated properties
• Plan, implement, and encourage attendance for Community events
• Complete annual assessments and reassessments, including collecting updated proof of income for residents on the SHP and HOPWA caseload, and other applicable housing programs
• Meet and document interaction toward resident service goals no less than four (4) times a year
• Enter service notes and upload forms into Service Point within 72 hours of interaction with resident
• Assist required reports (i.e., APR, BHT) as required by management within designated time frames
• Coordinate the welfare of UHS residents identified as fragile and/or vulnerable for housing displacement due to poor housekeeping and/or physical/mental limitations
• Conduct resident interviews with property manager and RSS team
• Assist with property inspections with property manager
• Act as liaison between the client and property management to assist with resolving tenancy issues and provide eviction prevention services and assist with mediation sessions
• Provide services to address and resolve resident problems with macro issues (i.e., SSA, SSI, Medicare, physical and mental health issues)
• Assist tenants to identify and achieve their goals and meet their needs by providing access to various available services

• Make referrals and connections with appropriate community-based services such as but not limit to (physical/mental health, job readiness/training, aging, wellness, alcohol/drug abuse issues, financial, etc.)
• Work with the Manager, RSS team and Leasing Department on conducting random client file audits and program compliance
• Support residents in identifying and overcoming barriers to accessing resources
• Assist with form completion related to service needs as required and advocate on behalf of clients
• Assist in the coordination, training, and supervision of students and volunteers involved in any community outreach program on behalf of UHS
• Desire to work with underserved populations, particularly those with elevated risk and/or homeless populations and willingness to meet with clients where they are most comfortable
• Foster a success-oriented, accountable, collaborative, and ethical environment by complying with all UHS policies and procedures as outlined in the UHS Operations Manual and attending staff meetings as directed
• Regular, reliable, and prompt attendance to uphold the highly collaborative and mission-driven culture
• Represent UHS competently and professionally communicate and interact with colleagues and residents
• Perform other duties as assigned

 

 

Qualifications

KNOWLEDGE AND SKILLS: The requirements listed below are representative of the knowledge, skills and/or abilities required.
Education and/or Experience: Must be at least 18 years old. Associate Degree in social work or related field required. Bachelor’s degree preferred. Minimum of three (3) years’ experience working with underserved populations, previous work with elevated risk and/or homeless populations preferred.

Computer Skills / Technical Knowledge: General knowledge of computer applications for the frequent use of electronic mail, word processing, data entry, language translation cell phone app, spreadsheets, graphics, etc. Must be able to utilize the Service Point Data Management system.

Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, decimals, and percentages.

Language Skills: Ability to read and comprehend simple instructions, short correspondence, and memos in English. Ability to write simple correspondence and respond to questions. Interact professionally with managers, employees, clients, vendors and the public, particularly difficult/resistant patient populations and multiple types of providers in a variety of settings. Bilingual, including the ability to communicate in American Sign Language, strongly preferred.
