Job Summary
Provides care for the medical needs and assists with activities of daily living for children enrolled in the Skilled Nursing program.
Job Responsibilities
- Essential Duties and Responsibilities include the following. Other duties within the scope of practice may be assigned.
- Completes and documents initial assessments for new patients enrolled in the Skilled Nursing program.
- Provides daily head-to-toe assessment of assigned patients, including vital signs, and documents in electronic health record (EHR).
- Administers prescribed medications and treatments such as catheterizations, nebulizer treatments, chest physiotherapy, and oral and tube feedings in accordance with professional nursing standards, policies, and procedures, and documents administration in EHR.
- Maintains awareness of the comfort and safety needs of patients by assisting them with duties such as diaper changes, toileting, positioning, and making beds.
- Observe patients, and document significant events, conditions, reactions, and interventions in EHR.
- Notifies the supervisor and/or the patient’s physician of the patient’s condition and reaction to drugs, treatments, and significant incidents.
- Requests new orders from the patient’s physician in response to changes reported by the caregiver or changes in the patient’s condition.
- Responds to and coordinate team response to medical emergencies involving assigned patients, based on nursing standards, policies, and procedures.
- Provides and documents patient and family education.
- Participates in department quality improvement activities including, but not limited to, departmental training, routine staff meetings, and routine one-to-one meetings.
- Cleans work area and equipment as assigned and as needed throughout the day.
- Attends meetings and conferences and participates in other activities to promote professional growth.
- Offers guidance to Nurse Techs working with the same group of patients.

Education and/or Experience
Degree in Nursing from NLN accredited institution. One to two years of related experience and/or training desired. Pediatric experience preferred.
Certificates, Licenses, Registrations
Current, unencumbered Registered Nurse state license. CPR American Heart Association Basic Life Support (BLS) Provider with skills testing certification.
Computer Skills
Basic computer skills and ability to learn new software and web-based applications such as electronic health records (EHR).
