Job Summary

Provides care for the medical needs and assists with activities of daily living for children enrolled in the Skilled Nursing program.

Job Responsibilities

Essential Duties and Responsibilities include the following. Other duties within the scope of practice may be assigned.

Completes and documents initial assessments for new patients enrolled in the Skilled Nursing program.

Provides daily head-to-toe assessment of assigned patients, including vital signs, and documents in electronic health record (EHR).

Administers prescribed medications and treatments such as catheterizations, nebulizer treatments, chest physiotherapy, oral and tube feedings in accordance with professional nursing standards, policies and procedures, and documents administration in EHR.

Maintains awareness of comfort and safety needs of patients by assisting them with duties such as diaper changes, toileting, positioning, and making beds.

Observes patient, and documents significant events, conditions, reactions and interventions in EHR.

Notifies supervisor and/or the patient’s physician of patient’s condition and reaction to drugs, treatments, and significant incidents.

Assists with daily activities planned by Recreational Activity Team.

Collaborates with Lead Nurse or Nurse Coordinator to request new orders from patient’s physician in response to changes reported by caregiver or changes in patient’s condition.

Responds to and coordinates team response to medical emergencies involving assigned patients, based on nursing standards, policies and procedures.

Provides and documents patient and family education.

Participates in department quality improvement activities including, but not limited to, departmental training, routine staff meetings, and routine one-to-one meetings.

Cleans work area and equipment as assigned and as needed throughout the day.

Attends meetings and conferences and participates in other activities to promote professional growth.

Offers guidance to Nurse Techs working with the same group of patients.

Qualifications

Qualifications

Education and/or Experience

Degree in Nursing from NLN accredited institution. One to two years related experience and/or training desired. Pediatric experience preferred.

Certificates, Licenses, Registrations

Current, unencumbered Registered Nurse state license. Current American Heart Association Basic Life Support (BLS) Provider with skills testing certification.

Computer Skills

Basic computer skills and ability to learn new software and web-based applications such as electronic health records (EHR).
