Job Summary

Responsible for entering assessments and intake information into various databases on behalf of Permanent Supportive Housing departments. Responsible for assisting the Director of Permanent Supportive in running monthly reports, and audits, and maintaining a high level of data quality.

Job Responsibilities

Enters assessments and intake information into various databases on behalf of the Permanent Supportive Housing department. 

  • Utilizes and masters Park Center’s electronic health records system (e.g., SmartCare), HMIS, and any other necessary grant databases. 
  • Enters all applicable program data in a timely manner into EHR and HMIS
  • Assist with the creation and maintenance of data systems in collaboration with Permanent Supportive Housing, Homeless Services, and QI Department
  • Assist with the creation and maintenance of data systems in collaboration with Empower Tennessee. 
  • Attend the Permanent Supportive Housing department team meetings to support data quality and other data-related activities in EHR as needed
  • Attend meetings with Empower Tennessee to support data quality and other data-related activities as needed.

Assists Directors of Permanent Supportive Housing,  Homeless Services, Quality and Improvement, and IT in running monthly reports. Maintains a high level of data quality. 

  • Checks all databases and internal spreadsheets on a routine basis to maintain data quality and accuracy. 
  •  Implements procedures to improve data quality and data collection. 
  •  Reminds team to enter data into respective databases as needed. 
  • Coordinates with Empower Tennessee to ensure data is accurate before entering into respective databases.
  • Coordinates with Homeless Services and Permanent Supportive Housing departments to run audits and data reports from HMIS to submit to the Office of Homeless Services (OHS)
  • Serve as the point of contact for Park Center for HMIS for contact with OHS

Other Duties 

  • Complete internal audits for the Intake department to review intakes for completion
  • Assist the QI department with completing an internal audit form for Managers to use when auditing staff members’ documentation
  • Coordinating with IT to create and edit new forms and processes in HER
  • Working with the QI Department to improve the quality of data collection and reporting to stakeholders across Park Center
  • Attends necessary meetings regarding PSH services and advocacy including Office of Homeless Services meetings, Care Coordination meetings, etc. as needed. 
  • Adheres to all program policies and procedures. 
  • Completes all other duties as assigned.

Qualifications

  • Bachelor’s degree in human services-related field or at least 3 years of experience working in Human Services
  • Strong computer and data entry skills with great attention to detail. 
  • Experience working in HMIS within Davidson County is strongly preferred
  • Strong ability to work within a team. 
  • Strong time management skills and ability to independently meet deadlines. 
  • Strong communication skills. 
  • Ability to work in a professional and ethical manner. 
  • Must have proven written and verbal communication skills. 
  • Must have a valid driver’s license. 
  • Must be able to show proof of personal automobile insurance. 
  • Must be insurable under Park Center’s automobile insurance.
