Job Summary
The Program Manager is responsible for overseeing and executing the impactful programs of the Joe Beretta Heart Foundation (JBHF) in cities where JBHF has partner hospitals. This role ensures seamless service delivery that supports patients and families during medical crises. The Program Manager works closely with the Executive Director, staff, volunteers, and community partners to manage, grow, and evaluate programs, including but not limited to Emergency Housing, Papa Joe’s Plates, holiday outreach, and direct patient assistance.
Job Responsibilities
Key Responsibilities
Program Coordination and Implementation
- Emergency Housing: Coordinate temporary lodging support for families who must relocate for lifesaving care. Communicate with hospitals to identify eligible patients and ensure timely accommodations.
- Papa Joe’s Plates: Collaborate with nutritionists, chefs, and partners to provide culturally and regionally appropriate recipes for JBHF’s heart-healthy eating program. Work with partners to oversee the delivery of meals and educational resources.
- Holiday Outreach: Organize and execute seasonal programs to deliver gifts and meals to inpatient families during holidays. Coordinate logistics, volunteers, and hospital partnerships.
- Direct Patient Assistance: Administer programs providing financial and emotional support, including gas cards, utility assistance, and resource referrals. Conduct weekly check-ins and hospital visits as requested.
- Develop Future Programming: Podcast, Visitation, Mentoring, Support Group
Program Development and Evaluation
- Monitor program outcomes to ensure alignment with organizational goals and community needs.
- Develop and implement systems for tracking program metrics and participant feedback.
- Identify opportunities for program expansion and innovation in response to evolving patient and family needs.
- Collaborate with staff to secure funding for programs, providing programmatic data for grant proposals and reports.
Volunteer and Partner Engagement
- Recruit, train, and manage volunteers to support program delivery.
- Build and maintain relationships with hospitals, community organizations, and other partners to ensure program success.
- Check-in weekly with social workers at partner hospitals to ensure programs are being delivered seamlessly.
- Represent JBHF at community events and meetings to raise awareness about programs and services.
Administration and Reporting
- Manage program budgets, ensuring efficient use of resources.
- Prepare regular reports on program activities, outcomes, and challenges.
- Maintain accurate records of program participants and services provided.
- Secure patient testimonials and stories to highlight in JBHF marketing and fundraising efforts.

Qualifications
- Education: Bachelor’s degree in social work, nonprofit management, healthcare administration, or a related field preferred.
- Experience: At least 2 years of experience in program management, preferably in a nonprofit or healthcare setting.
- Strong organizational and time-management skills with the ability to handle multiple projects simultaneously.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Proficient in using technology, including databases, spreadsheets, and virtual communication tools.

About The Joe Beretta Heart Foundation
There are almost a million new cases of heart failure each year in the US, and many of those people will face the uncertainty, fear isolation, and darkness of advanced heart failure. JBHF partners with eight hospitals in two states to equip patients and families with mentors and materials about options and what to expect and provide emotional support in the hospital. Most live far away from treatment or need help recovering, so we assist with financial and short-term housing needs so they don’t have to worry and can receive the care they need.
