Job Summary

 

The Case Manager works directly with clients seeking re-entry from incarceration to assist with case-management and finding permanent housing.

Job Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Meet with each client once weekly for case management inside Rutherford County Correctional Facilities
  2. Teach weekly cognitive restructuring, healthy relationships, relapse prevention, etc. course inside the correctional facilities
  3. Ensures trauma-informed care model is implemented by conducting an initial, client-let meeting to establish rapport, build trust and gather pertinent information
  4. Maintains knowledge of and collaborative relationships with other programs in the county and state of Tennessee
  5. Establishes a transitional plane and treatment plan for all clients pre-release
  6. Observes progress by continually re-assessing needs; monitoring and evaluating goals and objectives set by client in their treatment and transitional plan and maintaining progress notes
  7. Meeting with team weekly to discuss prospective clients, progress/status reports on current clients
  8. Facilitating interdisciplinary approaches
  9. Analyzes therapeutic case records from clinicians, as needed, to identify and ensure goals, objectives, and community resources are documented and progress is monitored.
  10. Monitors cases by verifying clients’ attendance, observing and documenting any self-reported value and comfort level of clinical treatment while in the program, advocates for needed services and entitlements, obtains additional resources, intervenes in crises, and provides personal support
  11. Monitors and documents daily progress of significant changes in client’s behavior, achievements, or barriers in order to provide additional resources
  12. Communicates clients’ progress by participating in weekly Doors of Hope/VOCA team meetings and evaluations
  13. Prepares reports by collecting, analyzing, and summarizing programming outcomes and results
  14. Updates job knowledge by participating in educational opportunities and professional development through trainings and professional organizations
  15. Arranges all class schedules both pre-release and post-release
  16. Trains/interacts/assigns tasks with social work interns while they are going into the correctional facilities
  17. Perform other duties as assigned

Qualifications

 

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

  1. Bachelor’s Degree in social work, criminal justice or a related field
  2. Experience working with re-entry, substance use disorders, mental health, or co-occurring disorder preferred
  3. Previous case management preferred
  4. Previous experience working in a correctional facility preferred

OTHER SKILLS

  1. Excellent documentation skills.
  2. Possess a valid driver’s license with acceptable driving record
  3. A strong sense of and respect for confidentiality concerning participants and fellow employees.
  4. Ability to appropriately respond to crisis situations
  5. Effective Communication: Ability to effectively communicate through oral, written and technological means; actively listening and expressing oneself clearly, to ensure those that need specific information acquire it; plan and deliver communication that makes an impact and persuades the audience.
  6. Initiative and Self-motivation: Plans work and carries out tasks without detailed instructions; makes constructive suggestions; prepares for problems or opportunities in advance; undertakes additional responsibilities; responds to situations as they arise with minimal supervision.
  7. Stress Tolerance: Displays emotional resilience and the ability to withstand pressure on an on-going basis; Deals with difficult situations while maintaining performance; Seeks support from others when necessary and uses appropriate coping techniques
  8. Team Work: Works collaboratively with other to achieve goals and objectives; continually tries to improve team effectiveness; hold self and others accountable for team performance; motivates and encourages all members to work together in order to achieve maximum results; Supports group decisions and puts group goals ahead of own goals; effectively interacts with others
  9. Ability to speak, read, and write in English.
  10. Skills in Microsoft Word, Excel, and Outlook in order to keep records, communicate within and outside of the Agency and organize pertinent data and program materials.
