Job Summary

Summary: The MSW Social Worker/Case Manager will be responsible for evaluation and
delivery of social, emotional and other resources and assistance for people leaving
incarceration and coming to Tennessee Prison Outreach Ministry’s transitional housing
program; working with another case manager and other team members to support
transition home residents in their journey to emotional, spiritual and material self-
sufficiency; and eventually overseeing graduate-level interns from local universities who
wish to learn about incarceration and reentry by interning at TPOM.

Job Responsibilities

Essential Functions:

  • Conduct initial assessments of new residents and identify goals to be completed and resources needed; document these in treatment plan.
  • Conduct subsequent meetings to monitor progress and complete documentation in a timely manner.
  • Establish helping relationship with residents and help them with engagement of resources.
  • Respond in trauma-informed manner to situations as they arise.
  • Maintain ongoing communication with supervisor.
  • Coordinate with the interdisciplinary team to assure coordination of care and continuity of care delivery.
  • Demonstrate the ability to react to change productively and handle other essential tasks as assigned.
  • Consistently follow ministry policies and procedures.

  • Masters degree from an accredited school of social work.
  • Active social worker license in the state of Tennessee.
  • Knowledgeable about local social services and have experience with at-risk populations.
  • Ability to communicate well both verbally and in writing.
  • Excellent time management, organization, and task completion skills and must be flexible to schedule changes.
  • Must possess a valid driver’s license.
  • Must be computer literate with working knowledge of Google Suite or ability to learn quickly.
