The primary role of the Integrated Behavioral Health Care Coordinator is to administratively support and facilitate care management activities as assigned for behavioral health target patients and to assist the Integrated Behavioral Health Clinicians with telephonic follow-up calls and referrals to internal providers and community-based resources in supporting the best health outcomes possible for patients of UNHS.  Working as a team with the Behavioral Health providers and additional team members that support the patients the care coordinator is part of the whole person health approach team in support of better health outcomes.

Must be able to work in a Patient Centered Medical Home model. Must facilitate partnerships between patients, physicians and health teams with focus on care coordination and integration of treatment internally and externally. Must work diligently to assure that services are accessible, continuous, comprehensive, coordinated, compassionate, and culturally effective. Must be committed to eliminating barriers to care that is centered on the needs and convenience of patients above all other factors.

Job Responsibilities

1.  PATIENT CARE/CARE TEAM

a.  See all patients with problem assigned for management obtaining history and providing assessment as needed.

b.  Maintain a registry and tracking system for specified diagnoses, conditions or problems as assigned.

c.  Provide patient education materials, maintaining appropriate literature, and resources as guided by the BH Clinicians

d. Contact and follow up with patients regarding appointments, meetings, labs, etc.

e.  Communicate with providers regarding patient progress.

f.  Practice in collaboration with providers following protocols and guidelines.

g.  Maintain appropriate medical record keeping.

2.  PROFESSIONAL DEVELOPMENT:

a.  Devote time in developing and improving clinical and communication skills that enable her/him to manage patients in a high quality, cost effective and service-oriented manner.

b.  Share in clinical discussion, applying new information and knowledge to practice and actively imparting new clinical knowledge to colleagues.

c.  Know community resources.

3.  TEAM:

a.  Assure collegial harmony, demonstrating behavior consistent with UNHS’ vision, mission and goals; primary care centered; integrated inter-disciplinary care; teamwork; cooperation; effective communication with colleagues; and shared workload are a few of the team’s values.

b.  Utilize UNHS’ Organizational Improvement Plan and performance review to enhance delivery of high quality, cost effective care, participating in QA activities such as chart reviews, peer review, committee work, continuing education and in-service projects.

c.  Assists in development of similar team projects and collaboratives.

d.  Other tasks as assigned

Qualifications

QUALIFICATIONS:

1.  Bachelor’s degree in social work, psychology, counseling, or a similar field of study

2.  Minimally 2 years of experience in similar position preferred

3..  Knowledge of health disparity

4.  Sensitivity to population to be targeted for services

5.  Ability to teach and work with people

6.  Knowledgeable in professional field.

7.  Ability to work with diverse populations

8.  Commitment to UNHS mission and vision
