- Communicate any difficulties making visits with the supervisor as soon as the problem is discovered.
- Billing requirements must be met monthly (i.e. all visits must be completed at the right time and in the right location each month).
- Return phone calls and emails timely (i.e. within 24 hours.)
- Submit requested service amendments within 48 hours.
- Ensure that all needed documents/consents are thoroughly completed on the correct forms, signed, and turned in with the annual ISP.
- Notes must contain evidence of detailed, meaningful interaction with the person and progress towards completion of outcomes; show evidence of follow-up on issues of concern until resolved; be accurately and thoroughly completed; and turned in by the 5th of the following month. Any notes returned for correction must be corrected and returned to supervisors promptly.
- ISCs are expected to be punctual and have good attendance.
- ISCs are expected to assist when needed with Sub-visits.
- ISCs are expected to carry a full caseload not to exceed 40 people which will be assigned by their direct Supervisor.
- If an ISC’s caseload is below the expected cap of 40 their caseload will be considered “open” which means their caseload can be added to during the duration of their employment not to exceed 40 people.
Qualifications
- A Bachelor’s degree from an accredited college or university in a human services field.
- A Bachelor’s degree from an accredited college or university in a non-related field plus one and/or year of relevant experience.
- An Associate degree and/or two (2) years of relevant experience.
- High school diploma or general educational development (GED) certificate plus four (4) years of relevant experience.
- Excellent organizational skills
- Ability to coordinate and facilitate meetings
- Ability to work as a team member by always bringing issues, concerns, or pending actions to the attention of all other COS members so they can be discussed and resolved between team members.
- Ability to glean pertinent information from multiple sources and communicate that information effectively to all parties.
- Ability to communicate professionally in multiple written and verbal formats (i.e. emails, required documentation, phone calls, etc.)
- Excellent observational skills; Ability to think critically and plan strategically
- Excellent time management skills
- Excellent conflict resolution skills
- Ability to develop, nurture, and respect self-advocates and family member advocates.
- Familiarity with Tennessee’s Department of Intellectual and Developmental Disabilities services is a plus.
- Familiarity with Microsoft Word documents and templates is a plus.
- Strong typing skills are a plus.
