Job Summary

Essential Functions:

  • Maintain communication with Circles of Support (COS) regarding what is important to and what is important for the person supported. Ensure that important events that impact people are effectively communicated between staff, COS members, and to others important to the person.
  • Provide information about: services available through the HCBS waiver, TennCare MCOs, ECF and community resources; appeal rights; and choice of available qualified providers.
  • Provide information and assist in the Medicaid Waiver and / or TennCare appeals process when needed services are denied.
  • Advocate for people to:
  • actively participate in decision making.
  • live, work (if desired) and be involved in their communities.
  • be involved in meaningful relationships
  • be valued, respected and treated with dignity
  • exercise their rights
  • Coordinate transitions between services, service providers, homes, etc.
  • Gather information for ISP’s in a timely manner so that the information remains accurate, describes the supports needed, justifies the need for services, and requests authorization for DIDD services.
  • Send information for ISP’s to supervisor in a timely manner so that services can be requested.
  • Document all support coordination activities in accordance with established DIDD policies and The Arc’s practice.
  • Visit each person at least once a month; making sure that every third month a visit is done at home.
  • All employees are required to have a dependable car and valid insurance coverage as required by state law.
  • Assess risks and ensure that risks are communicated to all parties.
  • Promote the safety of people across all environments by maintaining communication with all involved regarding the health, safety and / or welfare of the person supported.
  • Immediately notify regional investigators, crisis pager, DHS and / or DCS of all allegations of abuse, neglect and mistreatment.  Notify the regional office and / or crisis pager within 4 hours for emergency room visits, unexpected hospital admissions, incidents involving law enforcement or mobile crisis, and incidents with potential for serious or negative consequences. Notify the regional office and / or crisis pager within 1 hour of any death.

  • Maintain the confidentiality of records, files and information concerning the individual and his or her family.
  • Communicate any difficulties making visits with the supervisor as soon as the problem is discovered.
  • Billing requirements must be met monthly (i.e. all visits must be completed at the right time and in the right location each month).
  • Return phone calls and emails timely (i.e. within 24 hours.)
  • Submit requested service amendments within 48 hours.
  • Ensure that all needed documents / consents are thoroughly completed on the correct forms, signed and turned in with the annual ISP.
  • Notes must contain evidence of detailed, meaningful interaction with the person and progress towards completion of outcomes; show evidence of follow up on issues of concern until resolved; be accurately and thoroughly completed; and turned in by the 5th of the following month. Any notes returned for correction must be corrected and returned to supervisors promptly.
  • ISCs are expected to be punctual and have good attendance.
  • ISC’s are expected to assist when needed with Sub-visits.
  • ISC’s are expected to carry a full caseload not to exceed 40 people which will be assigned by their direct Supervisor.

Qualifications

Skills Needed:

  • A Bachelor’s degree from an accredited college or university in a human services field.
  •  A Bachelor’s degree from an accredited college or university in a non-related field plus one (1) year of relevant experience.
  •  An Associate degree plus two (2) years of relevant experience.
  •  High school diploma or general educational development (GED) certificate plus four (4) years of relevant experience.
  • Excellent organizational skills
  • Ability to coordinate and facilitate meetings
  • Ability to work as a team member by always bringing issues, concerns or pending actions to the attention of all other COS members so they can be discussed and resolved between team members.
  • Ability to glean pertinent information from multiple sources and communicate that information effectively to all parties.
  • Ability to communicate professionally in multiple written and verbal formats (i.e. emails, required documentation, phone calls, etc.)
  • Excellent observational skills; Ability to think critically and plan strategically
  • Excellent time management skills
  • Excellent conflict resolution skills
  • Ability to develop, nurture and respect self-advocates and family member advocates.
  • Familiarity with Tennessee’s Department of Intellectual and Developmental Disabilities services a plus.
  • Familiarity with Microsoft Word documents and templates a plus.
  • Strong typing skills a plus.
