Job Summary

The Housing Retention Case Manager will be responsible for assisting clients who have recently obtained permanent housing, who are at risk of eviction, and to help navigate the journey from the streets to permanent housing. This position will ensure clients have the skills and tools needed to maintain their current housing or find other appropriate alternative solutions, especially those in crisis situations. The ideal candidate for this position will also use creative and collaborative techniques for housing retention and rapid rehousing to minimize the impact of homelessness on both individuals and families.

Job Responsibilities

• Making contact and building relationships with individuals and families who are at risk of (or who have a history of) experiencing homelessness while providing a quick turnaround on resources, referrals, and outreach support

• Creatively networking to find opportunities for housing and other critical services

• Working with those experiencing homelessness to develop and execute plans targeting program goals such as

  • To achieve greater housing stability
  • To obtain, maintain and/or increase income, and
  • To achieve greater self-determination through engagement with a holistic and case-specific array of supportive services leading to long-term success and preventing a return to the streets (including rehab, doctor visits, mental health treatment, housing, connecting to local resources, etc.)

• Provide case management and oversight for all clients (quickly and effectively overcoming obstacles, goal setting, weekly meetings, helping them build a support system, etc.)

• Assisting clients as they navigate application processes, obtain and provide necessary documentation for financial assistance and housing; evaluate applicant information against program eligibility criteria

• Helping eligible clients apply for and obtain disability or other income as applicable

• Maintaining consistent communication with clients both by phone and in person

• Effectively delegate responsibilities that are volunteer-appropriate and ensure follow-up

• Working with community partners and MCP volunteers to end the revolving-door of homelessness whenever and however possible

• Maintain a list of affordable community housing and available shelter space and keep MCP volunteers up-to-date on openings and availability of resources

• Attending weekly outreach and community meetings to effectively advocate for those we serve and encourage strong communication between outreach volunteers and community partners

• Keep up-to-date case notes in Charity Tracker (the community HMIS system) when working with clients and distributing donations

• Complete all monthly tracking and grant requirements in a timely manner

• Transporting individuals as needed

• Telling the story of the organization in an ethical and strengths-based way through managing social media accounts

• Other responsibilities as needed

Qualifications

• Bachelor’s degree in related field AND two years of direct service with individuals experiencing homelessness and poverty OR any equivalent combination of experience/education from which comparable knowledge, skills and abilities have been gained

• Demonstrated patience, kindness, and compassion for individuals who have experienced trauma and who may struggle with intensive mental and physical health needs, addiction, and behavioral health issues

• Alignment with Murfreesboro Cold Patrol’s Mission Statement and Core Values

• Valid driver’s license and insured vehicle

• Trustworthiness and transparency

• Experience in homeless outreach or homeless services and/or case management in a related field

• Self motivated and able to work independently while remaining organized and meeting deadlines

• Demonstrated competency in working with community partners and volunteers

• Knowledge of (or willingness to be trained in) evidence based practices including Trauma-Informed Care, Strengths-based approaches, and Motivational Interviewing

• Strong communication skills- written, verbal, and interpersonal communication

• Proficiency in basic computer skills and data tracking and management

• Detail-oriented

• Ability to collaborate well with others and be a strong team player

• Care and concern for the unity and well-being of the organizational collaborative
