Job Summary

Housing Navigator – West Nashville

The Housing Navigator is a full-time position which functions in the coordination and management of supportive services and resource navigation for guests of Community Care Fellowship experinecing homelessness.  This position will primarily work within our Mobile Housing Navigation Center for the West Nashville area. The Housing Navigator reports directly to the Associate Director and should be aligned with Community Care Fellowship’s mission and core values.  The Housing Navigator will work with the Resource Navigator to administer assessment tools, determine urgency of need, and directly connect individuals experiencing homelessness to resources to facilitate placement into short-term housing at the Mobile Housing Navigation Center, and then into permanent housing within 90 days.  

Job Responsibilities

The Housing Navigator is specifically responsible for these functions: 

  • Guest Services
    • Connect guests with outside agencies and community partners
    • Provide guidance and referrals and assist guests with expressed needs
    • Assist guests in obtaining documents necessary for permanent housing and income sources

  • Records, Meetings, & Administration
    • Maintain and report required data from grant funding requirements
    • Enter and maintain guests records in HMIS and CCF’s SalesForce database
    • Assisting with other staff tasks as needed, contributions to social media content, occasional help with fundraising efforts, engagement with volunteers and supporters, and a general spirit of gratitude, teamwork, and collaboration
    • Attend Board meetings and Nashville-Davidson Continuum of Care meetings as needed

 

  • Resource & Housing Navigation
    • Creatively networking resources to open up opportunities for housing and critical services
    • Coordinate services and advocate for the guests with other agencies and service providers
    • Making contact and building relationships with people experiencing homelessness while providing resources, referrals, and support
    • Navigating complex social services with people so they can obtain needed resources like identification documents, healthcare, food assistance, transportation, housing, and other needed services and supplies
    • Work with the Resource Navigator to help people move from encampments to permanent housing utilizing the Coordinated Entry System through HMIS
  • Program Coordination & Support
    • Work with the Resource Navigator to coordinate the Mobile Housing Navigation Center program in West Nashville through tenant referrals from the Metro Homeless Impact Division
    • Work with the Associate Director and other staff to coordinate the CCF Mobile Navigation Center program by identifying, enrolling, and supporting guests through resource and housing navigation
    • Support the Employment & Education Manager in coordinating the CCF Career Counseling Program by scheduling counseling appointments, helping guests complete necessary paperwork, and helping find long-term sustainable employment that leads to self-sufficiency

  • Training
    • Complete trainings as needed for continuing education and professional growth, such as:
      • Housing Navigator Training (Metro Homeless Impact Division) – required
      • HMIS Coordinated Entry Training (Metro Homeless Impact Division) – required

Qualifications

Bachelor’s Degree requied.
