Job Summary

Summary:  As the Health Navigator, you will work with individuals at risk of or living with HIV to provide medical, educational and social support and navigation throughout the care and prevention continuum. You will assist in increasing knowledge and awareness around HIV and PrEP/PEP, and ensuring access to testing, medical care and support services. You will be responsible for coordinating and implementing community HIV prevention activities, conducting risk reduction HIV/STI prevention education, performing HIV and STI testing, HIV treatment adherence, general GLBTQ health promotion support, navigation and life skills based coaching.   Contact with clients will occur both in the clinic setting and in the community. 

Job Responsibilities

Core Navigation Duties: 

  • Provide individualized HIV and STI prevention support, information and training.
  • Conduct education sessions on risk factors and strategies to reduce HIV and STI transmission.
  • Conduct HIV and STI testing and provide results and counseling.
  • Implements and utilizes Evidence Based Practices and program interventions such as ARTAS (Anti-Retroviral Treatment and Access to Services).
  • Teach others about managing their mental, physical and emotional health, especially related to GLBTQ specific health activities, screenings, and medical and behavioral health care.
  • Document contact with clients and those in care at clinic in monthly report documents ie; RedCap
  • Adhere to strict procedures of confidentiality, release of information and data security.
  • Develop and implement program marketing and outreach of prevention and biomedical interventions.

PrEP Navigation Duties:

 

  • Will oversee all PrEP related issues for clinic including but not limited to:
    • Assigning PrEP navigators equitably
    • Liaise between PrEP clients, Clinic and Pharmacies, as needed
    • Create and maintain database of PrEP patients for clinic.

 

  • Complete all tracking and reporting requirements for outcomes and evaluation
  • Maintains appropriate professional standards and provides appropriate follow-up for consumers
  • Provides appropriate linkage and referral to relevant services to optimize health outcomes
  • Provide pro-active follow-up after medical visits to ensure client satisfaction and understanding of follow-up medical care plan, referrals and physician instructions
  • Maintain accurate daily logs records, monthly outcome reports, and files for each client
  • Arrange transportation for clients to medical / support services appointments as deemed necessary
  • Work closely with inter-disciplinary teams (educators, case managers, other staff) to engage PrEP patients in appropriate services and resources and assist with coordination of care
  • Promote consumer engagement in therapy with our clinicians, engagement with psychiatric appointments, compliance with the healthcare registry, involvement and engagement with wellness services and engagement with primary care.

Qualifications

Required Qualifications:

  • Bachelor’s degree or equivalent experience.
  • Strong written, oral communication, public speaking and marketing skills
  • Understanding of social determinants of health and their impact on an individual’s ability to engage in and maintain health care resources.
  • High level of proficiency with Microsoft Office software (Word, Excel, Outlook).
  • Ability to utilize an electronic client record.
  • Ability to work with sensitive information and maintain confidentiality.
  • Ability to maintain an objective and compassionate attitude in dealing with varying types of situations and personalities.
  • Ability to work 40 hours a week with some evenings, holiday and weekend work.
  • Valid driver’s license, clean DMV record, reliable transportation and automobile insurance.

Preferred Qualifications:

  • Personal history as consumer of services for Gay Male/MSM populations.
  • Experience with and knowledge of HIV/AIDS, and the LGBTQ community.
  • Verbal and written fluency in Spanish and English.
  • Previous HIV field or community health center experience.
  • Spanish speaking ability is an added asset
