Job Summary

Job Purpose

The main purpose of the Community Health Worker (CHW) is to serve as an ambassador for health. A CHW has the skills and experience to understand what patients are going through and helps them get through difficult times. A CHW is someone who comes from the community he or she serves and will act as a caring neighbor to help patients address the social, medical, and community problems that lead to poor health. The goal is to help patients deal with the ‘real life’ issues that keep them from staying healthy, help with the tasks of getting medical care, and work on health goals by doing things like planning healthy meals or finding time to exercise.

 

Job Responsibilities

Primary Duties and Responsibilities

  • Participate in initial and ongoing training
  • Meet patients and conduct a needs assessment, including helping patients to set health goals
  • Make weekly follow-up calls and home visits to patients
  • Motivate patients to meet their health goals
  • Help patients with social issues like hunger, housing concerns, and employment challenges
  • Assist patients with organizing their records, making follow-up appointments, and filling prescriptions
  • Help patients fill out applications for benefits like insurance and food stamps
  • Run a weekly support and/or health education group for patients
  • Work with other CHW team members to create a directory of community resources
  • Document each patient encounter in detail within 24 hours
  • Prepare reports and documents as needed or requested
  • Attend regular team meetings
  • Help with the community health assessments, outreach clinics, and other special events
  • Other duties as reasonably assigned

 

Qualifications

Education and Experience

  • Resident of Nashville with good knowledge of the resources of this community
  • Experience helping and serving members of the community

Qualifications

  • Passionate about Christian ministry to the underserved and in full agreement with Siloam’s mission and core values
  • Willingness to serve persons with limited access to healthcare and who are from different cultures and faith beliefs
  • Positive attitude and desire to serve neighbors
  • Adherence to OSHA and confidentiality requirements
  • Ability to work well in a team setting
  • Interested in health issues
  • Respected in the community
  • Able to and comfortable with home visits (requires access to a car and possession of driver’s license)
  • Computer literacy and familiarity with MS Office products
  • Bilingual in English and Arabic/Spanish

Preferred Characteristics:

  • Part of the served community (lives in or near area served)
  • Creative and able to do a lot with few resources
  • Prior experience as outreach worker, case manager, or similar role a plus
