Job Purpose

The primary objective of the Community Health Program Coordinator is to organize and support implementation of Community Health grants and programs.  Strong organization strategies, tech-savvy and people skills are necessary to coordinate program activities and analyze data.  This position is grant-funded and may be contingent upon such funding.

Job Responsibilities

Primary Duties and Responsibilities

  • Analyze health system data to identify patients who are at high-risk for poor outcomes 
  • Coordinate patient enrollment in the Siloam CHW program
  • Collect, manage, and analyze data for CHW program 
  • Assist the Nashville Neighbors Program Manager in leading monthly Nashville Neighbors (NN) trainings 
  • Assist the NN Program Manager with administrative tasks as needed 
  • Work with NN Program Manager to enroll participants during RMS appointments
  • Coordinate and support implementation of COVID-response outreach in refugee and immigrant communities
  • Support future growth, adaptation, and scaling of community health programs, such as Community Health Worker programs, Nashville Neighbors and/or community/government-based partnership programs
  • Collect, manage, and analyze health system and program data to identify patients for enrollment in community health programs and, when relevant, directly enroll participants
  • Support the organization and coordination of regular trainings, meetings, and project calendar with team, community partners, and/or volunteers

Organizational Support

  • Implement administrative processes to optimize continual improvement
  • Participate in weekly team meetings and strategic planning sessions
  • Assist program leadership in administrative activities including assistance with CHW recruitment, hiring and training; grant reporting; and program evaluation
  • Ensure that program activities operate within the policies and procedures of the organization
  • Liaise across departments to coordinate effective and efficient program delivery
  • Other duties as reasonably assigned

 

Qualifications

Education and Experience

  • A bachelor’s degree is necessary
  • A master’s degree in public health or comparable is preferred
  • Familiarity with and experience in the following fields are strongly preferred: nonprofit, social sector or social impact fields; cross-cultural experience, particularly with immigrant and refugee populations; and Community Health Workers

Qualifications

  • Passionate about Christian ministry to the underserved and in full agreement with Siloam’s mission and core values
  • Excellent computer skills in Microsoft Office; statistical analysis skills a plus
  • Experience with data capture software; REDCap software skills a plus
  • Adherence to OSHA as well as confidentiality requirements
  • Ability to communicate effectively with persons from different cultural, linguistic and religious backgrounds
  • Bilingual skills a plus
  • Superior time management and organizational skills, including the ability to multitask with ease
  • Able to meet deadlines and remain calm under pressure
  • Works well with a wide variety of people, including all staff and partner organizations
  • Comfortable giving direction to others, asking questions and managing up
  • Excellent verbal and written communication skills
  • Able to manage projects independently and juggle multiple projects simultaneously
