Job Summary

The Manager is responsible for all aspects of the delivery of hospice services to patients/families in the home setting.  They are responsible for the coordination of the interdisciplinary teams and monitoring the expenses of the programs.   The Manager holds a unique position of leadership and must foster a positive attitude and maintain a sense of urgency in responding to referrals, admissions, and patient/family needs.  The Home Care Manager supervises the nursing staff and ancillary services.

ROLE

The Home Care Manager oversees all clinical staff, patient/family care, and coverage of these services 24 hrs/day, 7 days/week.  The position requires hiring staff and ensuring that all clinical staff are educated and trained to do the job.  They monitor regulatory changes and make adaptations as needed to maintain compliance, federal and state.  The Home Care Manager has a working knowledge of agency policies to maintain compliance or update them as needed.   The Home Care Manager performs staff evaluations according to policy.

Job Responsibilities

RESPONSIBILITIES/DUTIES (not limited to)

  • To supervise the planning, coordination, and delivery of patient/family care.
  • To interview, assess, and evaluate potential patients/families or delegate the responsibility to appropriate staff.
  • To assign each patient/family to the appropriate staff member(s) or delegate the responsibility to the appropriate staff.
  • To interview, evaluate, and hire potential clinical/ancillary staff.
  • To provide for regular interdisciplinary team meetings for the development, review, and revision of the patient/family plan of care, including the exchange of information, review of problems or unique issues from caseloads, and sharing of professional support.
  • To implement and monitor compliance with agency policies and procedures.
  • To provide for and conduct staff meetings for information and staff support as indicated.
  • To make visits to patients/families/facilities as indicated by patient/family/facility staff needs, for the purpose of supervision or education.
  • To supervise the maintenance of medical records, assuring accuracy, completeness, and compliance with licensing regulations, certification standards, and legal and ethical standards.
  • To provide verbal and written performance evaluations for assigned staff.
  • To monitor clinical expenses, including equipment, medications supplies, and labor.
  • To promote the utilization of resources for the benefit of team members, patients, and families consistent with agency procedures.
  • To identify the need for staff education and assist in the development of in-service programs for the interdisciplinary team and referral sources.
  •  To oversee orientation, training, or in-services of new nursing/ancillary personnel. Ensuring completion of new hire orientation, onsite visits, and competencies.
  • To participate in agency and community programs as requested to promote professional growth and understanding of hospice care.
  • Ensure compliance with federal/state regulations governing hospice care services by following policy and procedure consistent with agency standards.
  • To prepare and submit reports to the Chief Nursing Officer as requested.
  • Other duties as assigned.

RESULTS

  • Consistent 24-hour, 7-day nursing coverage of patients, families, and their needs.
  • Admission of patients to hospice care that are appropriate to receive services.
  • Maintenance of accurate and complete patient records.
  • Clinical staff that are educated in medical matters, as well as agency policies, and regulations.
  • Contented and knowledgeable community vendors.
  • Appropriate utilization of financial resources.
  • Adherence to agency policy, as well as federal and state regulations.

Qualifications

REQUIREMENTS (of Position)

  • Current RN license to practice in the state of KY.
  • A minimum of 3 years of clinical experience is required and a minimum of 2 years of hospice experience is preferred.
  • Excellent communication and interpersonal skills.
  • Basic computer skills.
  • Team player.
  • Organizational skills
  • Attention to detail and follow through.
  • Supportive and sensitive to staff and patient/family/facility needs.
  • Exercise judgment and discretion in confidential matters.

PHYSICAL DEMANDS and WORKING CONDITIONS

Requires sitting for long periods of time; occasional bending, stretching or lifting, and full range of body motion.  This position requires the ability to work under high-stress conditions for long periods of time.  Requires corrected vision, ability to hear a normal range of voice, drive, and negotiate steps, walkways, or housing not designed for handicapped access.  May require contact with individuals with communicable diseases and body fluids.  Occasionally, requires maneuvering items weighing over 100 pounds.  Able to work in a normal office environment, use office equipment, and prepare written reports.
