Job Responsibilities

What Will You be Doing?

  • Responsible for overseeing the CCM nursing team and the CCM program. Ensuring patients are being outreached to monthly, billed correctly, and patient needs are being met through the CCM program.
  • Lead the area team in initiatives and improvements and serve as a resource for the team.
  • Manage staffing needs, including time off requests and daily needs.
  • Support the CCM team and take initiative towards staff development and participate in required competency training and continuing education.
  • Work with multi-disciplinary team providing targeted case management and access community resources to improve and support physical, mental, and social needs.
  • Ensure caseload for each nurse is up to date and list of patients enrolled in the CCM program is up to date, processing monthly reports on department performance.
  • Ensure CCM nursing staff is following guidelines and billing care plans appropriately.
  • Facilitate enrollment of eligible chronic care patients into the CCM program.
  • Serve as a contact for CCM patients and families to request services, healthcare needs, and assisting with social determinants of health.
  • Collaborate with the primary care provider to update care plans monthly, ensuring goals and needs of patients are being met. Billing for the care plan as appropriate.
  • Coordinate patient specialist appointments internally within Mercy and externally.
  • Advocate for needed services on behalf of patients, such as assistance with medication costs or transportation barriers while utilizing community resources with an awareness of the assistance Mercy offers.

Qualifications

What We Need from You:

  • Graduation from an accredited Registered Nurse (RN) program, Bachelors preferred.
  • Licensed in the state of Tennessee as an RN.
  • At least 5 years of nursing experience required.
  • At least 2 years of case management required.
  • Basic Life Support (BLS).
  • Computer skills required for EMR.
  • Maintains standards of professionalism of Mercy Community Healthcare.
  • Excellent verbal and written communications skills.
