Job Summary

Matthew 25 is a Nashville, TN based non-profit that serves homeless men, including veterans, those out of incarceration, and other men experiencing homelessness. Matthew 25’s mission is to offer Housing, Help, and Hope. We offer Help in facing the issues causing homelessness. We offer Hope by walking alongside each of our clients, encouraging them in their recovery. Finally, we offer Housing for those committed to break free of homelessness, as they focus on themselves and their goals. 

Our Case Mangers work with the men in our program who are homeless.  They will conduct case management on a regular basis, ensuring all requirements of the program are being followed.  Matthew 25 Case Managers will also facilitate weekly psychoeducational groups.

Job Responsibilities

  • Facilitates the progress of formerly homeless men through case management to progressive or permanent housing. Orientation with new residents.
  • Interview and counsel clients to support them in following program policies which includes obtaining employment, saving money and paying program fees, attending required meetings, and doing their assigned chores.
  • Develop a comprehensive client-driven individual service plan with both short-term and long-term goals identified.
  • Complete a timeline and measures for each goal. Monitor progress towards goals in regularly scheduled weekly sessions.
  • Ensure the safety of all residents by implementing, monitoring, and enforcing the rules and regulations of communal living. This includes assisting with room checks and drug screenings as needed.
  • Evaluate and adjust individual service plans as needed and provide behavioral contracts and written warnings with consequences if satisfactory progress is not being met. Write and issue residents’ discharges.
  • Maintain client files to include one-on-one meetings, warnings, progress towards goals and documentation of any incidents.
  • Prepare regular and special reports from computer systems; may create reports, spreadsheets, and other formats using programmed software.
  • Establish and maintain office files; research and compiles information for files; purges files as required.
  • Refer clients to appropriate resources to assist with meeting goals. Follow up with resources on clients behalf when needed.
  • Report critical incidents immediately to the leadership team.
  • Attend to daily bed log, monitor reports, and termination reports in a timely manner in order to keep beds full.
  • Assure that specific VA grant program goals are consistently being met and provide extensive case work to foster transition from homelessness to permanent housing.
  • Participate in Matthew 25 team meetings, including liaison meetings, and outside community collaboration meetings.
  • Adheres to Matthew 25 policies, procedures, code of conduct and attendance rules.
  • Maintain strict confidentiality of all client information.
  • Uphold the mission of Matthew 25 by holding each resident to the program policies, as well as showing each resident respect, and the love and grace of God.

Qualifications

  1. Candidates must hold a degree in Social Work, Mental Health, Psychology, or a comparable degree from an accredited college or university.    
  2. Knowledge of VA voucher program and policies a plus.
  3. Knowledge of HMIS, Homeless Management Information System highly desired.
  4. Knowledge of motivational interviewing techniques for treatment success.
  5. Experience working with homeless individuals and those with substance use disorder
  6. Experience in the assessment for services and needs.

Other Requirements:

  • Possess proficient computer skills Outlook, Word, Excel, and PowerPoint.
  • Valid Driver’s license.
  • Lift up to 35 lbs.
  • Background and drug screening
