Job Summary

Be a part of a new and innovative program supporting Nashville’s LGBTQ young adults experiencing homelessness!  Launch Pad is looking for an experienced case manager to help our guests move toward housing and independence. 

Launch Pad, known for our LGBTQ-affirming emergency winter shelter, is now operating “scattered site” emergency housing utilizing shared apartment space  in Nashville. This is a much higher standard of care than a shelter environment can offer. Help us grow this exciting new program!

 

Job Responsibilities

 

Responsibilities include:

 

  • Handling a caseload of a maximum of 18 clients – though the case load will actually be a maximum of 12 for at least the next several months before we expand the program.
  • Being the first point of contact for new guests and entering their information into the city’s HMIS database. 
  • Doing intake for new guests
  • Supporting guests as they apply for replacement IDs and documents
  • Keep accurate and timely case notes for each guest
  • Connecting guests with mental health and physical health resources
  • Discussing and recording goals for guests and then tracking those goals and helping guests stay accountable for them
  • Distributing items of value such as bus passes
  • Helping guests keep track of and obtain renewals and med checks for prescriptions
  • Helping guests obtain and keep necessary appointments and keeping youth counselors aware of upcoming appointments to help remind guests
  • Participating in weekly staff meetings
  • Working with youth counselors to find ways to support our guests
  • Working a regular schedule with some flexibility for occasional urgent needs such as bringing in a new guest who is in an unsafe situation.
  • Providing basic housing navigation assistance for guests not otherwise being provided such assistance by other agencies
  • Assisting trans guests with legal name change procedures and with procuring gender affirming health services

Qualifications

 

The ideal candidate will have:

 

  • A proven track record as a strong LGBTQ ally and advocate, particularly with those in the trans community
  • A thorough understanding of and experience with the principles of trauma-informed care
  • Experience in case management, particularly with those experiencing homelessness 
  • Familiarity with mental health issues and mental health resources in the area
  • The ability to work independently with initiative and efficiency
  • Knowledge of housing resources and homeless resources in the area
  • A willingness to independently research and connect with new resources for our guests
  • A desire to help this exciting new program grow and improve over the coming years
  • The ability to exercise compassion and empathy while maintaining solid boundaries 
  • At least a bachelor’s in social work or related field though master’s level is preferred
  • Established practices for your own self-care

Benefits

Because we are not yet able to offer medical benefits, we will add a 7% stipend to salary to help offset the cost of your own insurance.
