Summary: This position provides individualized, needs-driven patient-family centered, case management services to patients and their families, including connecting the patent and family to internal and community services that improve overall physical, mental and spiritual well-being.

Job Responsibilities

Schedule appointments for patients with providers. Document all relevant information including date, time and location of provider appointments 

Complete comprehensive needs assessment and develop individualized care plans based on the physician’s treatment plan aimed at meeting patient needs. Document assessments and care plans in the patient chart. Conduct reassessment of care and family needs as necessary, including their financial situation to ensure proper sliding scale is assigned. 

Assess internal and community resources and coordinate those into the care plan to ensure the proper fit with the patient and family needs.

Provide resources that patient can use to better understand their diagnoses, including resources they can reach out to with questions about their diagnosis and treatment options and maintain communications with the patient, family, and care team.

Attend and represent Mercy at community events and meetings. Gather information about community events and needs and bring that back to Mercy for possible participation and outreach.

Qualifications

Minimum Qualifications:

Education: BS in Psychology, Social Work, or other human service field required.  LPN, Registered Nurse or Bachelor Degree Nursing preferred. Must be willing to become a certified manager and receive training in and practice universal health precautions

Experience: One year of case management experience or internship in a primary care healthcare setting required, 2 years preferred. Experience in a team-based approach to care preferred. Must be proficient in MS Office with intermediate typing skills and patient EMR.
