FiftyForward has an excellent opportunity for a Care Manager in our Victory Over Crime Program.  This is a hybrid (minimum 3 days in office), full-time position with regular weekday hours, Monday – Friday, 8am – 4:30pm.  This position is grant funded.

In this role, you will provide assistance, advocacy, connection to and navigation of resources to enable older adults who have been affected by crime to remain independent and living at home in the community.

The ideal candidate will have a passion for service to older adults, have excellent verbal and written communication skills, the ability to quickly learn our mission, being able to flex between working independently and within a team, and most importantly, a passion for service to older adults.

Job Responsibilities

  • Solicit and screen referrals to the program.
  • Administer a comprehensive functional assessment to determine client eligibility.
  • Collaborate and consult with clients, service providers, family members and significant others to implement care plan.
  • Criminal Justice System Navigation – assist clients to understand and navigate the complexities of the criminal justice system. This includes but is not limited to escorting clients to necessary court dates.
  • Client Observation – is observant of abuse indicators and knowledgeable of how to make a report, written or verbal, to applicable authorities.
  • Safety Planning – assist in development and implementation of safety plans to help clients plan for their safety and prevent future harm.
  • Navigation of Resources – assist clients with connection to and navigation of community resources to help promote safety and stability.
  • Socialization – interact with clients to build rapport and learn of new or ongoing needs and concerns.
  • Transportation – transport clients to appointments.
  • Education – provide instruction in the skills necessary for independent living and safety including home safety, accessing nutritious food, preparation of healthy meals, personal cleanliness and hygiene, dealing with disabling conditions, managing finances including safely managing banking and paying bills, use of public transportation, information on how to access community resources and/or other skills which would help the client achieve the goals they have set.
  • Make accurate, detailed daily notes on activities and interactions with clients in FiftyForward’s online data base system, attend all meetings and workshops as required, work cooperatively with other staff and volunteers to meet client needs and to update client records.
  • Assist supervisor with record keeping and reporting requirements of the Office of Criminal Justice Programs (OCJP) contract and other funding sources as applicable.
  • Perform outreach functions to inform the community of the availability of the FiftyForward Victory Over Crime program to assist with referrals to the program.
  • Perform other duties as assigned.

Qualifications

Skills

  • Interest in and sensitivity to frail seniors and their caregivers
  • Effective and professional communication (verbal/written) and presentation skills
  • Ability to work independently and as a team member
  • Knowledge of community resources and medical and legal terminology preferred
  • Proficiency in time management, organization, and problem-solving
  • Proven ability to be flexible and work cooperatively with a wide range of constituents and stakeholders

Qualifications:

  • Passion to promote and uphold FiftyForward’s mission of supporting, championing, and enhancing life for those 50 and older
  • Bachelor’s degree in social services or related field
  • Criminal justice or social service training preferred
  • 1-3 years’ experience working in social services field, experience working in victim advocacy or with older adults preferred
  • A valid TN driver’s license including a good motor vehicle record, and appropriate automobile insurance
  • Must have reliable transportation
