The Next Door provides a continuum of evidence-based substance abuse and mental health services for women in an environment of faith and healing to restore hope and a lifetime of recovery.

 

Summary:

 

Provides supervision for the intensive care management services for TND, Inc. Treatment services delivered within a recovery-oriented system of care targeting the mental health and substance-related recovery needs of women and their families.

 

Job Responsibilities

Primary Responsibilities and Duties include the following:

  • Supervise Care Managers working in TND Nashville Residential Treatment Services.
  • Be the primary contact for care management services for TND Nashville.
  • Provide and/or coordinate trainings for care managers.
  • Facilitate bi-weekly (weekly if needed) meetings with direct reports to discuss care management needs and client services.
  • Ensure that the care managers are meeting requirements for licensure, accreditation, and managed care.
  • Ensure care management team facilitates new Client orientation for all new Clients each day.
  • Provide care management services to residential treatment clients.
  • Coordinate all necessary recovery and mental health care, along with associated supportive services for clients in community and within the facility.
  • Connect clients with community resources and sober living environments.
  • Works with therapist to develop initial discharge plan upon admission with input and feedback from client, family, significant others, and treatment team; updates discharge plan as needed.
  • Collaborate with the Clinical Treatment Team.
  • Develop a community resource guide for care managers by going out to develop relationships with other agencies that can provide after care services to our clients.
  • Provide psychotherapy and/or psychoeducational groups as requested by supervisor.
  • Collaborate with Admissions, Utilization Review, Family Services, Support Services, Medical Services and Behavioral Health to ensure cohesiveness of the program.
  • Co-lead weekly Check-in and Treatment team meetings.
  • Coordinate random alcohol and drug screening of clients in treatment programs.
  • Conduct random internal chart audits for each level of care in Treatment Services monthly.
  • Ensure Care Management team is completing timely and professional documentation to support each client interaction and coordination of services per the documentation guidelines of The Next Door, Inc.
  • Participate in a rotating on-call system as a member of the management team.
  • When possible, participate in task force, in-service, trainings, and other community educational networking opportunities to educate others about TND, Inc. programs and to promote partnerships and sponsorships whenever possible.
  • Additional responsibilities as assigned.

 

Qualifications

Education and Experience:

  • Master’s degree in counseling or social work preferred.
  • Experience providing care management directly with women who have substance use and/or mental illness is required.
  • Experience coordinating or managing in a substance abuse treatment setting is preferred.

 

 

Other Requirements:

  • Possess proficient computer skills Outlook, Word, Excel, and PowerPoint
  • Valid Driver’s license
  • Personal Automobile Insurance
